The utilization of non-lung organs from deceased donors with a positive polymerase chain reaction (PCR) for severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) at the time donation can be lifesaving, although safety this policy must assessed.This is nationwide, prospective study, reporting experience on SARS-CoV-2-positive between December 15, 2020 and May 31, 2022 in Spain.A total 69 patients received solid organ transplant (41 kidney, 18 liver, 8 heart, combined liver-kidney) obtained 32 SARS-CoV-2 PCR (four them cycle threshold value <30). All recipients tested negative were free disease 2019 (COVID-19) symptoms prior to transplantation. Nasopharyngeal swab turned 4 (5.8%) 3, 8, 11, 20 days after transplantation, though evidence did not support donor-derived COVID-19. Four kidney lost their grafts two died: one heart recipient due cardiogenic shock liver-kidney lung hypertension right failure. Graft losses patient deaths deemed unrelated donor status by treating teams. No other adverse reactions reported.This preliminary supports use than lungs PCR-positive donors, alignment previous series. However, impact infection upon quality should established future research.
