The motor stage of Parkinson's disease (PD) can be preceded for years by a prodromal characterized non-motor symptoms like REM sleep behavior disorder (RBD), hyposmia, and constipation. Here, we show that multiple stages idiopathic PD, including the pre-motor stage, stratified according to inflammatory responses stimulation peripheral blood mononuclear cells ex vivo. IFNγ isolated monocytes reveals increased stimulation-dependent secretion TNF, IL-1β, IL-8 in PD relative moderate PD. Additionally, T stimulated with CD3/CD28 co-stimulatory beads diminished proinflammatory cytokine early-moderate prodromal. Receiver operating characteristic curves demonstrate several cytokines produced high predictive utility distinguishing individuals from neurologically healthy controls. Moreover, immune deficits CD8+ T-cell mitochondrial health being positively correlated IL-8, IL-10 subjects. Dysregulated may contribute inflammation progression, vivo stimulation-based assays have potential reveal novel biomarkers patient stratification progression endophenotypes.
