Background UK Biobank is a well-characterised cohort of over 500 000 participants including genetics, environmental data and imaging. An online mental health questionnaire was designed for to expand its potential. Aims Describe the development, implementation results this questionnaire. Method expert working group questionnaire, using established measures where possible, consulting patient group. Operational criteria were agreed defining likely disorder risk states, lifetime depression, mania/hypomania, generalised anxiety disorder, unusual experiences self-harm, current post-traumatic stress hazardous/harmful alcohol use. Results A total 157 366 completed questionnaires available by August 2017. Participants aged 45–82 (53% ≥65 years) 57% women. Comparison self-reported diagnosed with contemporary study shows similar prevalence, despite respondents being higher average socioeconomic status. Lifetime depression common finding, 24% (37 434) meeting use met 21% (32 602), whereas other less than 8% participants. There extensive comorbidity among syndromes. Mental disorders associated high neuroticism score, adverse life events long-term illness; addiction bipolar affective in particular deprivation. Conclusions The represents very large survey itself, presented here show face validity, although caution needed because selection bias. The genetic dissection of major depressive disorder (MDD) ranks as one the success stories psychiatric genetics, with genome-wide association studies (GWAS) identifying 178 risk loci and proposing more than 200 candidate genes. However, GWAS results derive from analysis cohorts in which most cases are diagnosed by minimal phenotyping, a method that has low specificity. I review data indicating there is large component unique to MDD remains inaccessible phenotyping strategies majority identified approaches unlikely be loci. show inventive uses biobank data, novel imputation methods, combined interviewer cases, can identify contribute episodic severe shifts mood, neurovegetative cognitive changes central MDD.
