Hair loss disorders pose a substantial global health burden, affecting millions of individuals and significantly impacting quality life. Despite the widespread use approved therapeutics like minoxidil finasteride, their clinical efficacy remains limited. These challenges underscore pressing need for more targeted effective therapeutic solutions. This review examines latest innovations in hair drug discovery, with focus on small-molecule inhibitors, biologics, stem cell-based therapies. By integrating insights from molecular mechanisms leveraging advancements research methods, development next-generation holds potential to transform management disorders. Future should prioritize antibody therapy treatments, as these approaches offer unprecedented opportunities address limitations existing options. Antibody therapies enable precise targeting key pathways involved follicle regulation, providing highly specific interventions. Similarly, therapies, including cell transplantation dermal papilla regeneration, directly regenerative capacity follicles, offering transformative restoration.

In recent years, there has been a dynamic development in therapies utilizing extracellular vesicles (EVs) including exosomes. Therefore, we have conducted an analysis of the scientific literature to verify current state knowledge about these therapies. A total 12 clinical studies were analyzed, covering use EVs treating skin aging, acne scars, alopecia, and wound healing. The results indicate that exosomes hold potential regenerative therapies, offering innovative non-invasive therapeutic approaches. At same time, significant challenges related standardization their production lack large-scale randomized identified. Thus, also evaluated investigated trials regard MISEV (Minimal Information for Studies Extracellular Vesicles) criteria. This review provides comprehensive overview contemporary applications therapy medicine, highlighting directions further research.

Formed through the inward budding endosomal membrane, exosomes facilitate transfer proteins, lipids, nucleic acids, including microRNAs (miRNAs), thereby influencing behavior function recipient cells. These vesicles are secreted by various cell types, keratinocytes, crucial for maintaining skin homeostasis, regulating immune responses, promoting wound healing. Exosomes demonstrated therapeutic potential addressing conditions such as hair disorders, cancers photoaging enhanced regeneration reduced oxidative stress. However, they also implicated disease progression, with pathogens utilizing exosome release to evade host responses.
