ABSTRACT Background Doxepin (DX) is used orally to relieve itching but can cause side effects like blurred vision, dry mouth, and drowsiness due its antimuscarinic effect. To reduce these adverse improve skin permeation, DX being developed in topical formulations. This study aims absorption by developing a microemulsion (ME) formulation (ME‐DX). Method Materials ME formulations containing 5% were prepared using the phase diagram method with mixture of essential oil (oleic acid), surfactant (Tween 80 Labrazol), cosurfactant (propylene glycol or PG). The physicochemical properties assessed application DX. Results droplet sizes ranged from 9.8 61.6 nm, viscosities between 114 239 cps. Studies showed significant correlation release percentages viscosity. ME‐DX‐2 released 46.51% drug after 24 h. Patients with chronic cholestatic liver diseases often experience itch and struggle this symptom. We discuss the mechanism of in patients diseases, such as primary biliary cholangitis (PBC) others, their therapies, including ileal bile acid transporter (IBAT) inhibitors. In PBC, there are high serum/plasma concentrations multiple factors, salts, bilirubin, endogenous opioids, lysophosphatidic (LPA), autotaxin, histamine. Bile LPA, autotaxin affect mediators skin sensory nerves, while opioid balance affects spinal cord. Itch is sensitized by both peripheral central nervous systems. Both mechanisms involved disease. Although IBAT inhibitors have been approved for use pediatric conditions, progressive familial intrahepatic cholestasis Alagille syndrome, inhibition seems to be a promising treatment refractory PBC. A traditional non-systematic review results narrative review. Multidisciplinary cooperation, involving hepatologists, dermatologists, pharmacists, could provide better PBC suffering from itch. conclusion, we summarized existing knowledge on caused especially focus therapies. Chronic pruritus (CP), attention-deficit/hyperactivity disorder (ADHD), and skin picking (SPD) are medical conditions that involve both somatic psychosocial dimensions, posing unique challenges in clinical management. While CP SPD often observed together, the link between ADHD these is less recognized. This conceptual work describes three women who suffered from a complex interplay of CP, ADHD, treated at our specialized bi-disciplinary psychodermatological clinic. Based on observation narrative review literature, we assume bidirectional, triangular relationship SPD.
