ABSTRACT Background Doxepin (DX) is used orally to relieve itching but can cause side effects like blurred vision, dry mouth, and drowsiness due its antimuscarinic effect. To reduce these adverse improve skin permeation, DX being developed in topical formulations. This study aims absorption by developing a microemulsion (ME) formulation (ME‐DX). Method Materials ME formulations containing 5% were prepared using the phase diagram method with mixture of essential oil (oleic acid), surfactant (Tween 80 Labrazol), cosurfactant (propylene glycol or PG). The physicochemical properties assessed application DX. Results droplet sizes ranged from 9.8 61.6 nm, viscosities between 114 239 cps. Studies showed significant correlation release percentages viscosity. ME‐DX‐2 released 46.51% drug after 24 h. Selected ME‐DX demonstrated Weibull, Log Wagner, zero‐order kinetics, improved permeability. ME‐DX‐8 exhibited an eightfold increase J ss P parameters compared control group. MEs maintained 99.9% 6 months without color changes separation, indicating long‐term stability. Conclusion research demonstrates that altering content composition change permeability characteristics when introduced into rats. Additionally, shows promise as effective vehicle for delivery atopic dermatitis treatment.

Neuropathic itch is a type of chronic pruritus resulting from neural dysfunction along the afferent pathway. It often accompanied by abnormal sensations such as paresthesia, hyperesthesia, or hypoesthesia. This condition, which may involve motor autonomic damage, significantly impacts patients' quality life, causing severe and associated comorbidities depression, disrupted sleep, social strain. accounts for 8% cases, though this be underestimated. comprehensive review focuses on nerve impingement primary pathophysiological mechanism various forms neuropathic including brachioradial (BRP), notalgia paresthetica (NP), anogenital itch. BRP, seen in middle-aged white women, manifests dorsolateral forearms typically exacerbated ultraviolet (UV) exposure related to cervical spine pathology. NP, prevalent presents upper back due thoracic compression. Anogenital pruritus, affecting 1-5% adults, linked lumbosacral issues after ruling out dermatologic conditions lichen sclerosus simplex chronicus. The pathophysiology involves both peripheral central mechanisms, with damage being key factor. Diagnosis requires thorough history, physical examination, potentially imaging studies. Topical agents menthol, capsaicin, lidocaine are used mild while systemic medications gabapentin, pregabalin, antidepressants prescribed moderate cases; however, no US Food Drug Administration (FDA)-approved therapies currently exist specifically Understanding underlying appropriate therapeutic strategies crucial managing effectively.

Patients with chronic cholestatic liver diseases often experience itch and struggle this symptom. We discuss the mechanism of in patients diseases, such as primary biliary cholangitis (PBC) others, their therapies, including ileal bile acid transporter (IBAT) inhibitors. In PBC, there are high serum/plasma concentrations multiple factors, salts, bilirubin, endogenous opioids, lysophosphatidic (LPA), autotaxin, histamine. Bile LPA, autotaxin affect mediators skin sensory nerves, while opioid balance affects spinal cord. Itch is sensitized by both peripheral central nervous systems. Both mechanisms involved disease. Although IBAT inhibitors have been approved for use pediatric conditions, progressive familial intrahepatic cholestasis Alagille syndrome, inhibition seems to be a promising treatment refractory PBC. A traditional non-systematic review results narrative review. Multidisciplinary cooperation, involving hepatologists, dermatologists, pharmacists, could provide better PBC suffering from itch. conclusion, we summarized existing knowledge on caused especially focus therapies. This provides therapeutic options diseases.

Chronic pruritus (CP), attention-deficit/hyperactivity disorder (ADHD), and skin picking (SPD) are medical conditions that involve both somatic psychosocial dimensions, posing unique challenges in clinical management. While CP SPD often observed together, the link between ADHD these is less recognized. This conceptual work describes three women who suffered from a complex interplay of CP, ADHD, treated at our specialized bi-disciplinary psychodermatological clinic. Based on observation narrative review literature, we assume bidirectional, triangular relationship SPD. To support this assumption, propose two hypotheses: (1) neurodevelopmental hypothesis, emphasizing an underlying disorder, case, might present with symptoms like dysfunction sensory processing, impulsivity, attention deficits as shared features reinforce SPD, (2) neuroinflammatory suggesting similar signatures promote co-occurrence In addition, provide specific suggestions derived experience how to manage patients combination conditions. Elucidating help develop personalized treatment strategies improve outcomes.

Vulvovaginal itching is a common yet often under-recognized condition affecting women across all age groups. Despite its prevalence, many dermatologists receive minimal training in vulvar diseases, leading to delayed diagnoses and prolonged discomfort for patients. This review explores the broad spectrum of causes, including infections, inflammatory conditions, neuropathic disorders, systemic illnesses. The complexity vulvovaginal pruritus requires multidisciplinary approach accurately diagnose treat. Contributing factors such as hormonal changes, personal hygiene practices, environmental exposures must also be considered. Treatment strategies typically begin with lifestyle modifications topical therapies, corticosteroids antifungals, but can extend medications biologics resistant cases. Additionally, nonpharmaceutical options sitz baths psychological interventions crucial managing chronic symptoms. However, there remains significant gap research, particularly regarding characterization female-specific long-term impact on quality life. some advances, available studies largely focus isolated causes rather than holistic nature condition. Further research urgently needed develop comprehensive, evidence-based guidelines diagnosing treating itching, that has profound effect both physical emotional well-being.
