Antimicrobial stewardship programs (ASPs) and diagnostic (DSPs) are essential strategies for effectively managing infectious diseases tackling antimicrobial resistance (AMR). These can have a complementary impact, i.e., ASPs optimize use to prevent resistance, while DSPs enhance accuracy guide appropriate treatments. This review explores the current landscape of in pediatric care, focusing on key factors, influencing their development, implementation, evaluation across various settings. A multidisciplinary approach is necessary, involving multiple healthcare professionals support comprehensive practices care. Objectives: We aimed to summarize the current state of antimicrobial stewardship (ASP) and diagnostic programs (DSPs) implemented in pediatric intensive care units (PICUs). Methods: Embase, MEDLINE, Scopus Cochrane Library were searched, including studies from 1 January 2007 20 February 2024. Studies included review if they assessed implementation an ASP or a DSP PICU. Identified references downloaded into Rayyan software, data extracted using standardized collection form. Results: 18 included; 13 described intervention, 5 intervention. Most retrospective adopted persuasive strategy for ASP, reporting positive effects on consumption. However, dramatically heterogeneous terms intervention type, outcomes metrics used, limiting possibility broader comparison. Diagnostic mainly impact biomarkers pathogen testing panels without significant antibiotic prescription patterns. Antimicrobial resistance changes not by majority studies. Conclusions: PICUs is still limited, with variability used evaluate outcomes. To enhance effectiveness these programs, it crucial harmonize allow adequate comparison results find best strategies inform PICUs.
