Autologous fat grafting is increasingly used in plastic, reconstructive, and esthetic surgery. Cryopreservation offers a promising solution for the long-term storage of adipose tissue, enabling multiple sessions while minimizing patient discomfort associated with repeated liposuction harvesting. This systematic review aims to analyze current literature focusing on factors that influence outcome cryopreservation, including use cryoprotectants, cooling warming rate, temperature, enrichment cryopreserved grafts. A search PubMed/MEDLINE database up November 2024 was performed, original preclinical clinical studies written English describing cryopreservation unprocessed or mechanically processed tissue (macrofat, microfat, nanofat). Eligible articles needed describe applied protocol, at least temperature. Studies adipose-derived stem cells (ASCs), stromal vascular fraction, microvascular fragments, other isolated components were excluded. Data rates, eventual supplementation frozen collected. Of 679 records identified, 59 met inclusion criteria. Adipose -80°C cryoprotectant, controlled slow cooling, fast represented most often protocol encouraging outcomes maintaining survival histological structure. Several indicated ASCs improves survival. Taken together, existing present diverse, some extent, conflicting results regarding protocols their effects viability. Hence, ideal autologous remains be established. Moreover, tailored may necessary derivatives, such as nanofat.
