Chronic viral infections like HIV, HBV, and HCV establish persistent interactions with the host immune system, resulting in evasion long-term dysfunction. These viruses use a range of strategies to limit defenses, such as downregulating MHC class I, disrupting interferon signaling, altering apoptosis pathways, suppressing cytotoxic T-cell activity. Key proteins, including HIV Nef, HBV X protein, NS5A, interfere antigen presentation JAK/STAT thereby reducing antiviral responses. induce exhaustion due exposure, which leads expression inhibitory receptors PD-1 CTLA-4 on T cells. Viral epigenetic changes, N6-methyladenosine modifications histone deacetylation, enhance by modulating gene infected Viruses further manipulate cytokine networks promoting an immunosuppressive environment through IL-10 TGF-β secretion, suppress inflammatory responses inhibit activation. This review examines molecular/cellular mechanisms that enable chronic escape immunity, focusing antigenic variation, disruption, control apoptotic pathways. It also addresses how genetic factors, HLA polymorphisms, influence disease progression. Lastly, we discuss host-targeted therapies, checkpoint inhibitors, treatments, CRISPR.

Extracellular vesicles (EVs) are lipid bilayer nanoparticles naturally released from cells, playing a crucial role in intercellular communication. They modulate gene expression and regulate physiological pathological processes, including acute lung injury (ALI) respiratory distress syndrome (ARDS). Research has shown that EVs contain variety of active components, biocompatible small size, do not trigger immune rejection, making the infusion exogenous promising therapeutic tool. With further research, engineering strategies have been proposed to enhance clinical potential EVs. These involve modifying either donor cells secrete or themselves can be engineered circumvent limitations native In this review, an overview biological properties is provided current treating ALI/ARDS, along with latest research findings, summarized. The challenges opportunities for translation as novel tool also discussed, offering new insights into treatment ALI/ARDS using EV technology.

Intestinal ischemia-reperfusion (I/R) injury is a common and severe clinical issue. With high morbidity mortality, it burdens patients the healthcare system. Despite efforts in medical research, current treatment options are unsatisfactory, urging novel therapeutic strategies. Oxidative stress dysregulated autophagy play pivotal roles pathogenesis of I/R injury, damaging intestinal tissues disrupting normal functions. The aim this study to fabricate macrophage membrane-coated manganese dioxide nanospheres loaded with rapamycin [Ma@(MnO₂+RAPA)] for alleviating injury. We engineered honeycomb MnO2 coated membrane act as drug delivery system, encapsulating RAPA. In vitro OGD/R model IEC-6 cells vivo mouse models were used. Targeting ability was evaluated through in-vivo imaging Effects on cell viability, reactive oxygen species (ROS) levels, generation, inflammatory factors, apoptosis, autophagy, biocompatibility detected by methods such MTT assay, fluorescence microscopy, ELISA kit, TUNEL Western blotting histological analysis. study, Ma@(MnO₂+RAPA) efficiently deliver RAPA damaged exhibited good ROS-responsive release. Our data showed that reduced ROS, increased O₂, inhibited inflammation, promoted while reducing apoptosis cells. model, significantly Chiu's score, improved tight conjunction proteins, decreased levels cytokines oxidative stress. released from Ma@(MnO₂+RAPA), enhanced expression autophagy-regulated proteins p62, Beclin-1, LC3II. safety confirmed analysis biochemical detection mice. results demonstrated alleviated stress, promoting inhibiting inflammation. This offers potential strategy

Ganoderma lucidum (Leyss. ex Fr.) Karst, commonly known as Lingzhi, has long been employed in traditional Chinese medicine for its medicinal properties, particularly alleviating respiratory issues like cough and asthma. Recognized both a therapeutic agent an edible supplement, Lingzhi is celebrated health-promoting benefits. Despite widespread use, the effectiveness of G. treating pneumonia not extensively studied, highlighting need further research. This research aimed to evaluate potential treatment uncover mechanisms behind effects, specifically examining how active constituents influence inflammatory pathways. The study utilized approaches such network pharmacology, bioinformatics, molecular docking, vivo experiments. High-performance liquid chromatography (HPLC) chromatography-mass spectrometry (LC-MS) analyses revealed eight triterpenoids lucidum, with ganodermanontriol being most prominent. Molecular docking studies anticipated interactions between these compounds target proteins, while experiments on pneumonia-induced rat models assessed efficacy ganodermanontriol. Additionally, HPLC LC-MS confirmed presence ethanol extract predominantly Network pharmacology identified key genes-including TNF, EGFR, ESR1, HIF1A, HSP90AA1, SRC-that played significant roles regulation In results demonstrated that mitigated lung tissue damage rats experimentally induced by reducing release mediators. Further mechanistic showed downregulated TNF-α inhibited NF-κB/MAPKs signaling These findings suggested holds promising anti-inflammatory targeting TNF/NF-κB/MAPKs pathway, offering novel approach.
