Vascular dementia (VaD) is a prevalent form of cognitive impairment with underlying vascular etiology. In this review, we examine recent genetic advancements in our understanding VaD, encompassing range methodologies including genome-wide association studies (GWAS), polygenic risk scores (PRS), heritability estimates, and family for monogenic disorders revealing the complex heterogeneous nature disease. While optimal cardiovascular health (CVH) has been linked to a lower risk of dementia, few studies considered individuals' genetic background. We aimed examine the interaction between CVH and predisposition on dementia among individuals with atherosclerotic disease (ASCVD). included 30,818 ASCVD patients from UK Biobank. was assessed using Life's Essential 8, determined by score (GRS) incorporating 85 variants. Cox proportional hazard models were used estimate ratios (HRs) for all-cause Alzheimer's (AD), vascular (VaD). Over median follow-up 13.5 years, 1,360 cases identified, including 489 AD 440 VaD cases. Higher levels associated reduced (HR high vs. low CVH: 0.60; 95 % CI: 0.47-0.77) 0.32; 0.19-0.54), stronger association in GRS. An integrative polygenic risk score (iPRS) capturing the neurodegenerative and vascular contribution to dementia could identify high-risk individuals improve prediction.
