Abstract INTRODUCTION We investigated the associations of longitudinal synaptic loss and cognitive decline with tau burden plasma biomarkers in Alzheimer's disease (AD). METHODS Twenty cognitively impaired (CI) individuals 16 healthy controls (HC) underwent biomarker assessments, amyloid positron emission tomography (PET), PET, density PET; after 1 year, PET were repeated. The relationships among burden, biomarkers, density, cognition investigated. RESULTS CI group had more synapse deposition than HCs. Longitudinal was positively associated decline, negatively deposition. Plasma glial fibrillary acidic protein (GFAP) mediates relationship between loss. Tau phosphorylated tau181, GFAP could predict decline. CONCLUSIONS increases pathology Highlights Cognitively medial temporal lobe, increased widespread neocortex controls. change
