Abstract Background Benzo[a]pyrene (B[a]P), a carcinogen pollutant produced by combustion processes, is present in the western diet with grilled meats. Chronic exposure of B[a]P hepatocellular carcinoma (HCC) cells promotes metastasis rather than primary proliferation, implying an unknown mechanism B[a]P‐induced malignancy. Given that exosomes carry bioactive molecules to distant sites, we investigated whether and how mediate cancer‐stroma communications for toxicologically associated microenvironment. Method Exosomes were isolated from stimulated BEL7404 HCC (7404‐100Bap Exo) at environmental relevant dose (100 nmol/L). Lung pre‐education animal model was prepared via injection cytokines. This study categorizes pollutant-induced inflammation–cancer transition into three stages: non-alcoholic fatty liver disease (NAFLD), fibrosis, and hepatocellular carcinoma (HCC). It systematically reveals the temporal heterogeneity of damage. The findings indicate that pollutants not only directly damage hepatocytes but also modulate key cells in immune microenvironment, such as hepatic stellate (HSCs) Kupffer cells, thereby amplifying inflammatory fibrotic responses, ultimately accelerating progression HCC. Mechanistically, early stage primarily cause hepatocyte injury through oxidative stress lipid metabolism dysregulation. Background The pre-metastatic niche (PMN) represents the microenvironment established in target organs before primary tumor metastasis, playing a crucial role organ-specific metastasis. Understanding and preventing PMN formation holds promise for enhancing immunotherapy efficacy reducing cancer-related mortality. Despite significance of this field, comprehensive bibliometric analysis is lacking. This study aims to identify global research trends hotspots through systematic evaluation, providing foundation future advancements field. Methods Publications related from 2005 2024 were retrieved Web Science Core Collection database. Bibliometric analyses visualizations conducted using VOSviewer, CiteSpace, Microsoft Excel, ArcGIS, Scimago Graphica, Charticulator. Results included 1,303 publications authored by 7,955 researchers 1,627 institutions across 62 countries, with articles published 400 journals. ABSTRACT Lung metastasis remains a leading cause of cancer mortality, driven not only by primary tumors but crucially dynamic interactions within the premetastatic niche (PMN). Emerging evidence reveals that organ‐specific PMN formation precedes tumor cell arrival, creating an immunosuppressive microenvironment conducive to metastatic colonization. Primary tumor‐derived factors interact with lung resident cells, promoting recruitment bone marrow‐derived cells and favorable for migrating cells. Despite progress, systematic integration PMN's multilayered regulatory mechanisms—from mechanism clinical application lacking. The review summarizes recent studies investigating effects in entire process metastasis. Our discussion revolves primarily around extracellular vesicles, matrix remodeling, proinflammation, immunosuppression, angiogenesis, metabolic reprogramming. We emphasize impact various treatment strategies on practice, while also summarizing current diagnostic methods exploring new therapeutic possibilities PMN. Finally, we propose roadmap future investigations into PMN, highlighting prioritized research axes bridge mechanistic discoveries bench‐to‐bedside translation PMN‐targeted interventions.
