Abstract INTRODUCTION We developed a multimarker blood test result interpretation tool for the clinical dementia practice, including phosphorylated (P‐)tau181, amyloid‐beta (Abeta)42/40, glial fibrillary acidic protein (GFAP), and neurofilament light (NfL). Abstract Staging the severity of Alzheimer’s disease pathology using biomarkers is useful for therapeutic trials and clinical prognosis. Abstract INTRODUCTION This study aimed to investigate the differential roles of various plasma biomarkers in a stepwise diagnostic strategy for Alzheimer's disease (AD). METHODS A total 2984 participants, including 666 cognitively unimpaired (CU), 2032 with clinical syndrome (ACS), and 286 non‐ACS individuals, were recruited. Plasma amyloid beta (Aβ) 42/40, four phosphorylated tau (p‐tau) epitopes, glial fibrillary acidic protein (GFAP), neurofilament light chain (NfL) levels measured using immunoassays. RESULTS NfL demonstrated fair excellent accuracy differentiating from CU groups (area under curve [AUC], 0.79 0.94). p‐tau217 had highest identifying Aβ (AUC 0.94) positron emission tomography status 0.91). In ACS group, was strongest predictor cognitive decline (p < .001). DISCUSSION may serve as useful screening tool, while is particularly valuable confirming AD pathology prognosis. Abstract INTRODUCTION Patients with dementia Lewy bodies (DLB) may have Alzheimers disease (AD) pathology that can be detected by plasma biomarkers. Our objective was to evaluate biomarkers of AD and their association positron emission tomography (PET) amyloid tau deposition in the continuum DLB, starting from prodromal stages disease. METHODS The cohort included patients isolated rapid eye movement (REM) sleep behavior disorder (iRBD), mild cognitive impairment (MCI‐LB), or a concurrent blood draw PET scans. Recent technological advances have improved the sensitivity and specificity of blood-based biomarkers for Alzheimer's disease related dementias. Accurate quantification amyloid-ß peptide, phosphorylated tau (pTau) isoforms, as well markers neurodegeneration (neurofilament light chain [NfL]) neuro-immune activation (glial fibrillary acidic protein [GFAP] chitinase-3-like 1 [YKL-40]) in blood has allowed researchers to characterize neurobiological processes at scale a cost-effective minimally invasive manner.
