The emerging role of extracellular vesicles (EVs) in central nervous system (CNS) diseases is gaining significant interest, particularly their applications as diagnostic biomarkers and therapeutic agents. EVs are involved intercellular communication secreted by all cell types. They contain specific markers a diverse cargo such proteins, lipids, nucleic acids, reflecting the physiological pathological state originating cells. Their reduced immunogenicity ability to cross blood-brain barrier make them promising candidates for both In context CNS diseases, have shown promise isolable from different body fluids, providing non-invasive method diagnosing monitoring disease progression. Extracellular vesicles (EVs) are nanosized, membrane-bound structures that have emerged as promising tools for drug delivery, especially in the treatment of lysosomal storage disorders (LSDs) with central nervous system (CNS) involvement. This review highlights unique properties EVs, such their biocompatibility, capacity to cross blood-brain barrier (BBB), and potential therapeutic cargo loading, including enzymes genetic material. Current therapies LSDs, like enzyme replacement therapy (ERT), often fail address neurological symptoms due inability BBB. EVs offer a viable alternative, allowing targeted delivery CNS improving outcomes. Abstract Extracellular vesicles (EVs) are emerging as critical mediators of intercellular communication in the tumor microenvironment (TME), profoundly influencing cancer progression. These nano-sized vesicles, released by both and stromal cells, carry a diverse cargo proteins, nucleic acids, lipids, reflecting dynamic cellular landscape mediating intricate interactions between cells. This review provides comprehensive overview biogenesis, composition, functional roles EVs cancer, highlighting their significance basic research clinical applications. We discuss how cells manipulate EV biogenesis pathways to produce enriched with pro-tumorigenic molecules, explore specific contributions key hallmarks such angiogenesis, metastasis, immune evasion, emphasizing role shaping TME driving therapeutic resistance.
