Abstract Mycetoma is a chronic suppurative granulomatous infection of the deep dermis and subcutaneous tissue prevalent in tropical subtropical regions caused by filamentous aerobic bacteria (actinomycetoma) or true fungi (eumycetoma), representing 60% 40% cases worldwide, respectively. The causative organism enters into tissue, usually foot, from contaminated soil vegetative material through inoculation thorn prick, repeated trauma. It commonly affects men, farmers field workers. Differentiating eumycetoma actinomycetoma can be challenging but required before starting prolonged treatment. One our patients presented with lesions on thigh sporotrichoid pattern that atypical, while other two were treated antifungal medication for years without proper investigation improvement. Early diagnosis mandatory to prevent destruction, bone invasion ultimate loss function investigative workup, histopathology direct microscopy discharge. We here report three clinical microbiology profiles successfully tablets trimethoprim-sulfamethoxazoleand amoxicillin–clavulanic acid along folic as proposed Cochrane systemic review protocol 2018.

ABSTRACT Mycetoma is a chronic granulomatous infection predominantly affecting young individuals, characterized by painless swelling and often misdiagnosed due to its resemblance other conditions. This case report presents the first documented instance of eumycetoma located in buttock 44‐year‐old male patient from Somalia, region with limited literature on this disease. The exhibited 10‐year history indurated swelling, which progressed discharge dark granules, leading diagnosis black grain eumycetoma. Comprehensive diagnostic methods, including imaging histopathological analysis, were used, confirming fungal etiology. Surgical excision followed, alongside antifungal therapy, resulting significant improvement. emphasizes necessity for increased awareness consideration mycetoma clinical practice, especially atypical presentations underreported geographical areas.
