Abstract Background Heavy metal exposure has been associated with various diseases. The purpose of this study was to determine the single and combined effects between blood heavy metals cognitive function in older adults. Methods This observational utilizes data from National Health Nutrition Examination Survey (NHANES) 2011 2014 involved 1460 participants aged ≥ 60 years. concentrations lead (Pb), cadmium (Cd), mercury (Hg), selenium (Se), manganese (Mn) were measured using inductively coupled plasma mass spectrometry (ICP-MS). Cognitive assessed immediate Recall test (IRT), delayed recall (DRT), animal fluency (AFT), digit symbol substitution (DSST). Linear regression, restricted cubic spline (RCS), subgroup analyses used assess relationship function. Results Multiple linear regression analysis showed a negative correlation Cd comprehensive scores (β=-2.698, 95% CI: -4.842, -0.554). Blood Se levels positively correlated (β = 0.049, 0.022, 0.076) all four domains. RCS that dose-response association appeared be (P-nonlinear 0.566), while exhibited parabolic shape tilted left 0.013). Conclusions A high concentration is strongly decline. However, protective effect on plateaus after reaching certain concentration. These findings may have significance for development interventions aimed at alleviating impairment.
