Abstract “Gluing” together integrated Geriatric Clinical Service lines (GCSL) within the US healthcare system is a significant challenge. Reasons encompass health professional workforce shortages, inconsistent requirements for geriatric educational competencies among disciplines, preconceived ageist attitudes about older adults with complex illnesses, and infrastructure that not aligned longitudinal interdisciplinary care needs adults. This review focuses on three major characteristics of have impeded widespread dissemination GCSLs: (1) US's historical fee service (FFS) reimbursement system; (2) increasing reliance upon disease specific specialty services patients resulted from advances in medicine; (3) rising consolidation systems over last 30 years. Three options are also provided might help change current future trajectories local political advocacy to implement policy legislation; expand physician by nontraditional means; reprioritize expansionist corporate behavior. Each these interventions will be hard achieve, but it time unite if GCSLs thrive as pathways improve outcomes medical, cognitive neuropsychiatric disorders.

Hearing loss, a global health burden, is closely associated with depression and sleep disorders. However, the combined effects of duration on hearing loss risk remain unclear. Data from China Health Retirement Longitudinal Study (CHARLS) were analyzed, including 6,374 adults aged 45 older. Cox proportional hazards models assessed relationship between depression, duration, loss. Mediation analysis explored potential mediating roles CESD-10 score new-onset Subgroup analyses by age, sex, BMI also conducted. Over 7-year follow-up, 1,422 participants developed Both short high scores independently increased Participants long but had ratio (HR) 1.59 (95% CI: 1.35, 1.87) for showed that mediated 10.1% association while 70.8% This study highlights significant interconnected in development Interventions addressing both may offer more effective strategies preventing managing

Background Existing literature indicates that physical activity has a significant positive impact on the emotions in older adults; however, mechanism of relationship between and remains unclear. This study aims to explore role social capital among adults. Methods First, mood indicators elderly were constructed through Euclidean distance method, descriptive statistics correlation analyses performed relevant variables involved this paper. Secondly, structural equation (SEM) was used establish parallel mediation model analyze mediating effect elderly. Meanwhile, order test potential influence gender path, Multi-group SEM divide samples into two groups males females, compared path coefficients Likelihood Ratio Test (LRT). Finally, average treatment ATT, i.e., experimental group, obtained propensity score matching test. Result The results show that: (1) Physical (2) Social (i.e., individual collective capital) significantly positively influences (3) only capital, analysis reveals plays Conclusion not directly enhances adults, but also exerts an indirect by strengthening capital. Therefore, it is recommended Chinese government promote connections interactions adults providing public spaces, organizing community activities, interaction platforms. These efforts could help improve contribute achievement healthy aging goals.

Atrial fibrillation (AF) is a common cardiac arrhythmia that significantly impacts the cardiopulmonary function and quality of life patients. Despite various treatment strategies, non-pharmacological interventions, particularly exercise have gained attention in recent years. Through systematic review meta-analysis, this study explores impact physical activity on capacity AF It assesses safety, clinical outcomes, physiological mechanisms intervention AF. The individual patient data (IPD) meta-analysis method were employed, following PRISMA-IPD guidelines, for literature selection, extraction, assessment. analysis focused patients randomized controlled trials. A total 12 trials involving 287 included. Meta-analysis demonstrated significant improvement 6-minute walk test (SMD=87.87, 95% CI [42.23, 133.51]), static heart rate (SMD=-7.63, [-11.42, -3.85]), enhancement (SMD=2.37, [0.96, 3.77]) due to exercise. There was also (SMD=0.720, [0.038, 1.402]). Exercise has effect improving with atrial fibrillation. Particularly, high-intensity training more capacity, emphasizing importance personalized plans enhancing health Further research needed explore effects future. CRD2023493917.
