Hyperacusis is a frequent auditory disorder where sounds of normal volume are perceived as too loud or even painfully loud. There high degree co-morbidity between hyperacusis and tinnitus, most patients also have but only about 30-40% tinnitus show symptoms hyperacusis. In order to elucidate the mechanisms hyperacusis, detailed measurements loudness discomfort levels (LDLs) across hearing range would be desirable. However, previous studies reported LDLs for restricted frequency range, e.g., from 0.5 4 kHz 1 8 kHz. We measured audiograms in 381 with primary complaint full standard audiometric 0.125 On average, had mild high-frequency loss, more than third tested ears thresholds (HTs), i.e., ≤20 dB HL. were found significantly decreased compared normal-hearing reference group, average values around 85 HL range. receiver operating characteristic analysis showed that LDL neither sensitive nor specific enough serve single test was moderate positive correlation HTs (r = 0.36), tended higher at frequencies loss present, suggesting unlikely caused by HT increase, contrast which main trigger. Moreover, our finding frequencies, regardless pattern indicates might due generalized increase gain. Tinnitus on other hand thought neuroplastic changes not share common mechanism.

The primary aim of this research was to evaluate the validity, efficacy, and generalization principles underlying a sound therapy-based treatment for promoting expansion auditory dynamic range (DR) loudness. basic therapy principles, originally devised hyperacusis among patients with tinnitus, were evaluated in study target sample unsuccessfully fit and/or problematic prospective hearing aid users diminished DRs (owing their elevated audiometric thresholds reduced tolerance). Secondary aims included: (1) delineation contributions from counseling components full-treatment protocol and, turn, isolated effects each these individual intervention success; (2) characterization respective dynamics full, partial, control treatments. Thirty-six participants bilateral sensorineural losses DRs, which affected actual or perceived ability use aids, enrolled completed placebo-controlled (for therapy) randomized clinical trial. 2 × factorial trial design implemented without various assignments therapy. Specifically, assigned randomly one four groups (nine per group), including: group 1-full achieved scripted plus binaural generators; 2-partial placebo generators (PSGs); (3) 3-partial alone; (4) 4-a neutral PSGs alone. Repeated measurements categorical loudness judgments served as outcome measure. categorical-loudness 1, measured at termination, significantly greater than corresponding pretreatment baseline 500, 2,000, 4,000 Hz. Moreover, increases "uncomfortably loud" (∼12 dB over 500 Hz) superior those either partial-treatment 3 4. Efficacy, assessed by treatment-related criterion ≥ 10 uncomfortable loudness, full (82% efficacy) compared that partial treatments (25% 40% combined alone, respectively) (50%). majority 1 improvements within months beginning treatment. effect much counseling, statistically indistinguishable most our analyses are valid have general applicability expanding DR individuals losses, who may often report aided problems. positive virtual isolation, respectively; however, delivery both approach essential an optimum outcome.

Hyperacusis is a frequent auditory disorder that characterized by abnormal loudness perception where sounds of relatively normal volume are perceived as too loud or even painfully loud. As patients show decreased discomfort levels (LDLs) and steeper growth functions, it has been hypothesized hyperacusis might be caused an increase in neuronal response gain the system. Moreover, since about 85% also experience tinnitus, conditions common mechanism. However, mechanisms give rise to have remained unclear. Here we used computational model system investigate candidate for hyperacusis. Assuming proportional summed activity all nerve fibers, was tuned reproduce perception. We then evaluated variety potential determining their effects on equal-loudness contours comparing results LDLs with hearing thresholds. best accounted nonlinear central Good fits average patient were obtained general affected frequency channels same degree, frequency-specific high-frequency range. needed applied after subtraction spontaneous nerve, which contrast current theories tinnitus generation based amplification activity. therefore different changes processing

Background: Hyperacusis is the difference in dB between a normal threshold of audibility for stimulus and level at which that becomes intolerably loud. Some persons with autism exhibit symptoms hyperacusis possibly attributable to phobias auditory stimuli. Desensitization procedures have been used as treatment appear promising patients hyperacusis. Methods: Our participant was 26 year old male previously diagnosed Asperger's syndrome who presented low-pitched voices concomitant vocal disorder manifesting mainly high pitch. Despite not fully accepting goals he agreed our clinic. A systematic desensitization program employed treat hyperacusis, including use recorded voices, musical instruments, environmental sounds. In addition, modified re-education therapy also included instruction on easy onset phonation, biofeedback, ergonomic progressive imagery relaxation techniques home exercises reduce hyperfunction promote phonatorily efficient breath-stream management. Results: Initial, 7 month, 13 month measures voice evaluations were conducted. showed participant's lower-frequency tones improved by up 30 bilaterally his running speech 40 right ear 35 left ear. His pitch, measured fundamental frequency, from 404.471Hz 280.78Hz. Conclusions: decreased (VRT) lowered However, results did generalize beyond The latter may be unwillingness accept goals, serve caveat future clinicians consider prognosis prior making decisions.
