Exercise has well-characterized therapeutic benefits in the management of type 2 diabetes mellitus (T2DM). Most beneficial effects exercise arise from impact nuclear factor erythroid related factor-2 (Nrf2) activation glucose metabolism. Nrf2 is an essential controller cellular anti-oxidative capacity and circadian rhythms. The rhythm influenced by genes on its expression, where timing rhythmicity signaling, such that differential physiological effects. evening management, as lowering blood weight. mechanisms responsible for these have not yet been associated with influence activity. A better understanding exercise-induced signaling can improve our appreciation distinct morning exercise. This review hypothesizes morning, when level already at high levels, leads to hyperactivation decrease while evening, levels are nadir improves lowers increases fatty acid oxidation. Exploring activators rhythmic could also provide valuable insights into optimal their application, holding promise timed treatment diabetes.

Abstract Background In emergency departments (EDs), long wait times and overcrowding are major challenges, worsened by the pandemic's increased patient volumes demands. Lean methodology could offer a structured approach to reduce inefficiencies, improve care quality, support nursing staff. Aim of study: This study aims evaluate impact applying optimize post-pandemic within an ER setting. Methods utilized mixed-methods design in private hospital Egypt. Data collection involved three tools: voice process observation sheet, which tracked journeys 100 patients; customer interviews, conducted with 90 patients assess satisfaction waiting times; business held 64 staff members available resources. Additionally, cause-and-effect analysis was summarized A3 report, identifying key factors contributing extended times. Results The average time department 157.87 min, making up 77.7% total length stay. consultation phase accounted for longest delays, 92.46 min. Patient moderate (61.74%), while resources higher (71.09%), but only 53.1% were satisfied Key causes delays included non-compliance triage protocols (95.0%), lack pathways (90.3%), insufficient bed capacity (83.1%). An report proposed strategies enhance satisfaction. Conclusion highlights as challenge EDs, significantly impacting service outcomes, workload. Lean-based strategies, such standardized improved pathways, essential reducing enhancing both healthcare environment.

The increasing prevalence of metabolic syndrome is associated with major health and socioeconomic consequences. Currently, physical exercise, together dietary interventions, the mainstay treatment obesity related complications. Although exercise training includes different modalities, variable intensity, duration, volume, or frequency, which may have a distinct impact on several characteristics to syndrome, potential effects timing are yet be fully elucidated. Remarkably, promising results regard this topic been reported in last few years. Similar other time-based including nutritional therapy drug administration, time-of-day-based become useful approach for management disorders. In article, we review role discuss mechanisms that could drive metabolic-related benefits performed time-dependent manner.

Twenty-four hour rhythmicity in whole-body substrate metabolism, skeletal muscle clock gene expression and mitochondrial respiration is compromised upon insulin resistance. With exercise training known to ameliorate resistance, our objective was test if can reinforce diurnal variation metabolism men with In a single-arm longitudinal design, 10 overweight obese resistance performed 12 weeks of high-intensity interval recurrently the afternoon (between 14.00 18.00 h) were tested pre- post-exercise training, while staying metabolic research unit for 2 days under free-living conditions regular meals. On second days, indirect calorimetry at 08.00, 13.00, 18.00, 23.00 04.00 h, biopsies taken from vastus lateralis 08.30, 13.30 23.30 blood drawn least bi-hourly over 24 h. Participants did not lose body weight weeks, but improved composition capacity. Exercise resulted reduced 24-h plasma glucose levels, modify free fatty acid triacylglycerol levels. Diurnal modified by period genes showing an interaction (time × exercise) effect mRNA levels 13.00 increased without inducing variation. Twenty-four-hour energy expenditure remained unchanged. Future studies should investigate alternative strategies or types interventions (e.g. diet drugs aiming improving sensitivity) their capacity respiration. KEY POINTS: Insulin associated blunted flexibility respiration, disruptions molecular circadian clock. We hypothesized that modifies found inflexibility h persisted after whereas independent time day. Gene Per1-3 Rorα changed particularly close day which performed. These results provide rationale further differential impact differently timed treat defects manifest particular
