Integrated Management of Post-Myocardial Infarction Patients Using Genetic Analysis and Remote Control DOI

Coronary heart disease (CHD) remains a leading cause of mortality and morbidity worldwide, particularly due to complications following myocardial infarction (MI). Despite advancements in therapeutic strategies, the prognosis for MI patients varies significantly, necessitating comprehensive approach that integrates genetic non-genetic factors. This study addresses knowledge gap by evaluating influence polymorphisms factors on 12-month patients. Using cohort 250 post-MI patients, we conducted analysis AGT, ADRB1, APOE, LPL, CYP2C19 genes assessed their correlation with clinical outcomes using modern statistical methods. Our findings indicate significant associations between specific gene patient prognosis, highlighting importance personalized medicine. The results suggest incorporating into management can improve survival rates inform targeted interventions, offering novel enhancing care settings.

Cilostazol (CIL), a BCS class II antiplatelet aggregation and vasodilator agent, is used for cerebrovascular diseases to minimize blood–brain barrier dysfunction, white matter-lesion formation, motor deficits. The current work aimed develop optimize cilostazol-loaded spanlastics (CIL-SPA) nose-to-brain delivery overcome the low solubility absorption, first pass-metabolism, adverse effects. optimal CIL-SPA formulation was loaded into Phytagel® (SPA-PG), Poloxamer-407 (SPA-P407), chitosan (SPA-CS) gel bases characterized in terms of colloidal properties, encapsulation efficiency (EE%), mucoadhesive biopharmaceutical aspects. developed situ gelling formulations showed <300 nm average hydrodynamic diameter, <0.5 polydispersity index, >|±30| mV zeta potential with high EE% (>99%). All met droplet size-distribution criteria nasal requirements (<200 µm), all adequate mucoadhesion properties. Both BBB-PAMPA horizontal permeability study through an artificial membrane revealed that had higher CIL flux cumulative at vitro conditions compared initial CIL. drug-release released ca. 100% after 2 h. Therefore, could be promising improving bioavailability delivery.

Nano-drug delivery systems provide targeted solutions for addressing various drug challenges, leveraging nanotechnology to enhance solubility and permeability. Liposomes, explored several decades, face hurdles, especially in oral delivery. Bile-acid stabilized vesicles (bilosomes) are flexible lipid vesicles, composed of phospholipids or other surfactants, along with amphiphilic bile salts, they show superior stability pharmacokinetic behavior comparison conventional vesicular (liposomes niosomes). Bilosomes skin penetration, fluidize the stratum corneum, improve stability. In applications, bilosomes overcome drawbacks, offering improved bioavailability, controlled release, reduced side effects. Vaccines using demonstrate efficacy, intranasal, inhalation, ocular, buccal applications delivery, targeted, efficient, activities. Formulations vary based on active substances optimization techniques, showcasing versatility potential across diverse routes. Therefore, aim this comprehensive review was critically explore state-of-the-art therapeutic applications.
