Puerarin injection is used for the treatment of chronic heart failure (CHF). The objective this study to investigate efficacy and safety puerarin as an adjunct therapy CHF through a meta-analysis systematic review. We conducted comprehensive literature search across eight databases, including PubMed, Embase, Web Science, Cochrane Library, up 1 March 2025, identify required randomized controlled trials (RCTs). Subsequently, we assessed included studies according principles outlined in Handbook performed using RevMan 5.4 Stata 12.0. A total 29 RCTs were included, comprising 2,480 patients, with 1,251 group 1,229 control group. demonstrated that combined conventional medication significantly improved cardiac function parameters patients compared pharmacotherapy alone, left ventricular ejection fraction (MD = 6.22, 95% CI [3.11, 9.33], P < 0.01), output 0.45, [0.35, 0.55], stroke volume 3.29, [2.02, 4.57], while reducing end-diastolic diameter -0.83, [-1.24, -0.42], 0.01). combination both increased effectiveness rate (RR 1.26, [1.21, 1.31], 0.01) hemodynamic parameters, along favorable modulation oxidative stress markers evidenced by elevated superoxide dismutase, glutathione peroxidase, catalase levels concomitant reduced lipid peroxidation malondialdehyde concentrations. This suggests adjunctive may provide benefits management, clinical outcomes, enhanced function, attenuated remodeling, optimized performance, stress, maintaining profile comparable therapy.
