Abstract Hippocampal atrophy on magnetic resonance imaging is an important biomarker in Alzheimer's disease (AD). While hippocampal was thought to result from tau tangles AD, different neuropathologies can lead atrophy, especially TAR DNA‐binding protein 43 (TDP‐43) pathology. In this narrative review, we evaluate existing studies the relative contribution of and TDP‐43 pathology medial temporal lobe (MTL) atrophy. We report a clear association both neuropathology with MTL even after correcting for other neuropathologies. Tau exhibits change in both spatial extent and density of pathology along the Alzheimer's disease (AD) spectrum with each aspect contributing to overall burden pathological tau. Nevertheless, studies using PET have measured either magnitude standardized uptake value ratios (SUVRs) or number Tau+ regions. Tau protein plays a pivotal role in maintaining neuronal structure and function through its regulation of microtubule stability polarity. Postmortem neuropathological examination, while the gold standard for diagnosing neurodegenerative diseases, often relies on limited regional sampling that may miss critical areas affected by Alzheimer's disease and related disorders. Ultra-high resolution postmortem MRI can help identify regions fall outside diagnostic criteria additional histopathologic evaluation. However, there are no standardized guidelines integrating histology in a traditional brain bank. We developed comprehensive protocol whole hemisphere 7T MRI-guided histopathological with whole-slide digital imaging analysis, providing reliable pipeline high-volume banking heterogeneous tissue. Our method uses patient-specific 3D printed molds built from MRI, allowing tissue processing permanent spatial reference frame. To facilitate pathology-MRI association studies, we created semi-automated to registration quantitative pathology scoring system using weakly supervised deep learning. validated this cohort of 29 brains diagnosis AD spectrum revealed correlations between cortical thickness phosphorylated tau accumulation.
