Abstract Telomeres and telomerase play crucial roles in the initiation progression of cancer. As biomarkers, they aid distinguishing benign from malignant tissues. Despite promising therapeutic potential targeting telomeres for therapy, translating this concept laboratory to clinic remains challenging. Many candidate drugs remain experimental stage, with only a few advancing clinical trials. This review explores relationship between telomeres, telomerase, cancer, synthesizing their as biomarkers reviewing outcomes completed We propose that changes telomere length activity can be used stratify cancer stages. Furthermore, we suggest differential expression components at subcellular level holds promise biomarker. From standpoint, combining telomerase-targeted therapies mitigate adverse effects inhibition may offer viable strategy.
