Immune dysregulation-induced inflammation serves as a driving force in the progression of metabolic dysfunction-associated steatohepatitis (MASH), while underlying cellular and molecular mechanisms remain largely uncharted. The gut microbiota plays a crucial role in metabolic dysfunction-associated steatotic liver disease (MASLD). Next-generation sequencing technologies are essential for exploring the microbiome. While recent advancements full-length 16S (FL16S) rRNA offer better taxonomic resolution, whether they establish stronger associations with risk of MASLD remains to be determined. This study utilized long-read FL16S and short-read V3-V4 profile microbiome compositions age-, sex-, BMI-matched case–control pairs obese children without MASLD. A random forest predictive model was employed, using gut-microbiota features selected based on top 35 most abundant taxa or linear discriminant analysis score greater than 3. model's performance evaluated by comparing area under receiver operating characteristic curve (AUC) through tenfold cross-validation method. Subjects exhibited significantly elevated serum alanine aminotransferase, triglycerides, homeostasis assessment insulin resistance levels compared controls. At genus level, detected both were similar, predominantly comprising Phocaeicola Bacteroides, followed Prevotella, Bifidobacterium, Parabacteroides, Blautia. AUC data (86.98%) higher that (70.27%), as determined DeLong's test (p = 0.008). demonstrates children, highlighting its potential real-world clinical applications.
