Apicoectomy versus apical curettage in combination with or without L-PRF application: a randomized clinical trial

This
study
compared
the
efficacy
of
apicoectomy
and
apical
curettage
with
without
leukocyte-
platelet-rich
fibrin
(L-PRF)
in
treating
large
periapical
lesions.
Sixty-four
adults
(30
male,
34
female)
a
previously
endodontically
treated
tooth
lesion
were
randomized
into
four
groups
(n
=
16):
retrograde
obturation
(RG),
orthograde
(OG),
RG
+
L-PRF,
OG
L-PRF.
All
participants
underwent
root
canal
retreatment
two
sessions.
The
group
root-end
resection
MTA
obturation,
L-PRF
application
to
bone
defect
following
protocol,
resection,
protocol.
Clinical
radiographic
assessments
performed
preoperatively,
at
1
week
1,
3,
6,
9,
12
months
postoperatively.
At
follow-up
visits,
pain
scores,
swelling,
mobility,
tenderness
percussion
(T-PER),
palpation
(T-PAL),
presence
fistula
clinically
assessed.
Periapical
radiography
determined
index
(PAI)
score
measured
area
(PALA).
Kruskal–Wallis
test
was
effect
single
independent
variable
(factor)
on
dependent
variable.
No
statistically
significant
differences
identified
between
for
preoperative
PAI
fistula,
T-PER,
or
T-PAL
(p
>
0.05).

lower
T-PER.
PALA

Hydroxyapatite

aimed
to
evaluate
histologically,
histomorphometrically,

microcomputed
tomography
an
implant
surface
coated
with
nanostructured
hydroxyapatite
(HAnano),

comparison

acid-etched
(DAA)
surface,
both
without
peri-implant
grafting
L-PRF,
installed
over-instrumented
sites

analysis

variance
(ANOVA)
Welch's
ANOVA
test
did
not
identify
significant
differences
between
groups.
On
average,
BIC
ranged
from
44%
(HAnano
+
L-PRF)
63%
(DAA
L-PRF).
µCT
revealed
that
volume
density
region
26%
39%
(DAA).
No
statistically
observed
Both
surfaces
studied
allowed
osseointegration
low
sites,
independently
8
weeks
implantation.
While
this
model
provided
controlled
conditions
evaluating
early-stage
osseointegration,
absence
functional
loading
relatively
short
follow-up
period
should
be
considered
when
extrapolating
findings
clinical
applications.
Future
studies
assess
these
variables
under
load-bearing
extended
observation
periods.

utilize

This
review
was
performed

Results:
In
total,
3416
title
abstracts
screened,
a
40
systematic
reviews
finally
included
present
umbrella
review.
Conclusions:
Research
supports
PRF
PRP
different
fields
is
huge
potential
for
patient
but
also
doctor
as
these
products
are
have
zero
cost.
However,
further
studies
needed,
especially
RCTs,
clearer
evidence
on
role
PRP.

Introduction:
