Nonsuicidal self-injurious behaviour (SIB) is a debilitating manifestation of physical aggression commonly observed across neurodevelopmental, psychiatric, and genetic disorders. This arises from multifactorial aetiology involving predispositions, epigenetic modifications, neurotransmitter dysregulation, environmental stressors. Dysregulation in dopaminergic, serotonergic, glutamatergic, GABAergic systems has been implicated the pathophysiology SIB, alongside structural functional abnormalities within fronto-limbic-striatal circuits. These disruptions impair key processes, such as emotional regulation, reward processing, behavioural inhibition, contributing to emergence reinforcement SIB. Advances preclinical research using genetic, lesion-based, pharmacological, animal models have instrumental elucidating molecular neurocircuitry underpinnings Emerging neuromodulation therapies targeting critical nodes network, particularly deep brain stimulation, shown promise treating severe, refractory SIB improving quality life. review integrates current evidence clinical studies, research, provide comprehensive overview therapeutic approaches. By focusing on mechanisms neural circuits underlying we highlight translational potential emerging pharmacological neuromodulatory therapies. A deeper understanding these pathways will pave way for precision-based interventions, bridging gap between applications related conditions.
