Abstract Pre-exposure prophylaxis (PrEP) is the administration of antiretroviral medications before HIV exposure to prevent infection. PrEP or biomedical prevention an essential part ending epidemic strategy. Currently, there are 4 guideline-approved dosing approaches PrEP, which include 2 oral formulations and 1 long-acting injectable. Unfortunately, most individuals who would benefit from do not receive a prescription, even fewer initiate continue PrEP. Barriers uptake persistence complex extend along socio-ecologic framework individual through structural. In this article, we highlight barriers care among priority populations, discuss evidence-based solutions, offer multilevel considerations for clinicians, researchers, community members increase access, uptake, in all.

In the United States (U.S.), Latina transgender women (LTW) are highly burdened by HIV and prioritized for pre-exposure prophylaxis (PrEP). This study explored intersectional barriers facilitators to PrEP uptake among LTW. Between February–November 2022, in-depth interviews were conducted with 27 LTW in LITE Study. Participants purposively sampled from 196 cohort based on (PrEP-naïve n = 8, PrEP-eligible not user 5, current 6, previous 8). We content analysis guided a Modified Social Ecological Model Intersectionality Framework. The mean age of participants was 32.3 (SD 12.9). Themes were: (1) Intrapersonal: Medical distrust, acceptability modalities, concerns about long-term health; (2) Interpersonal: Mistreatment healthcare, discrimination-related healthcare avoidance, difficulty finding trans-competent providers, language barriers, shame stigma; (3) Structural: context limited access gender-affirming care widespread silicone use, immigration status, economic marginalization, lack community outreach, transphobia anti-transgender legislative contexts, xenophobia. found multilevel influence persistence. Culturally tailored prevention efforts needed address LTW-specific provide information programs subsidizing PrEP, implement policy change ensure equitable access.

Using PRISMA technique, we conducted a review literature on women, transgender, PrEP. We used Google Scholar PubMed conduct search. Recent Findings Our initial search garnered 2,221 sources across two databases. However, n = 197 articles were included in this based screening process our inclusion criteria. Socio-cultural factors, provider awareness, limited awareness knowledge, communication, motivation, concerns about PrEP, barriers, agency, power, pleasure may impact utilization facilitators highlighted as well interventions, though limited. Background: Transgender and nonbinary populations are disproportionately affected by HIV face barriers to accessing HIV-related services. Pre-exposure prophylaxis (PrEP) may benefit those at risk of acquisition. However, PrEP awareness uptake, along with potential facilitators, have not been investigated among transgender individuals living in Canada. Setting: This study analyzed data from 1965 participants the 2019 Trans PULSE Canada survey, a national convenience sampling survey Methods: Data were estimate levels uptake identify predictors population. Prevalence ratios estimated block-wise modified Poisson regression models used assess awareness. Results: awareness, lifetime use, current use be 71.0%, 2.2%, 0.9%, respectively, full sample, 82.3%, 7.3%, 3.8% indications for use. Respondents who aged 45 years or older, transfeminine, Indigenous, Atlantic Quebec, had high school education less significantly likely aware PrEP. Lifetime sex work, past-year HIV/STI testing, being single nonmonogamous relationship, higher emotional social support positively associated Conclusions: There is need improve particularly revealed inequities within this population, which serve as targets future public health initiatives.
