Objectives: Hypercholesterolemia is a significant risk factor for cardiovascular disease and dyslipidemia. In 33–58% of individuals, current medications targeting 3-hydroxy-3-methyl-glutaryl-CoA reductase (HMGCR) low-density lipoprotein (LDL) receptor do not intend to reduce LDL cholesterol. Pancreatic lipase, responsible cholesterol absorption, can be potential target in hypercholesterolemia. The study aims conduct virtual screening molecular dynamics compounds derived from ethanolic extract Tiliacora acuminata leaves (TAL-EE) with pancreatic evaluate cell viability 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyl tetrazolium (MTT) assay, assess lipase inhibition, perform lipid accumulation assessment (Oil Red O staining) measure triglycerides (TG) HepG2 cells. Methods: Docking was carried out using AutoDock tools, interaction analysis performed PyMOL 2.0. Para-nitrophenyl butyrate porcine were used test TAL-EE inhibition. Viability (MTT test, IC50), Oil staining, measurements total (TC) triglyceride levels conducted cells treated TAL-EE. Results: 5α-Androstan-16-one cyclic ethylene mercaptole identified through have the highest docking score (−9.4 kcaL/moL) exhibiting stability dynamic studies. IC50 assay reported 20 μg/mL. After 48 h at lower doses, exhibited 67% reduction activity increased TG TC medium cells, indicating reduced intracellular levels. Conclusion: showed inhibition modulation, suggesting as an antihypercholesterolemic agent.
