Mucormycosis (previously called zygomycosis) is a diverse group of increasingly recognized and frequently fatal mycotic diseases caused by members the class zygomycetes. around 80 times more common in India, compared to other developed countries, with frequency 0.14 cases per 1,000 population. The most frequent causative agent mucormycosis following genera from Order Mucorales Rhizopus, Mucor, Rhizomucor, Absidia, Apophysomyces, Cunninghamella, Saksenaea. major risk factors for development are diabetic ketoacidosis, deferoxamine treatment, cancer, solid organ or bone marrow transplantations, prolonged steroid use, extreme malnutrition, neutropenia. clinical forms rhino-orbital-cerebral, pulmonary, cutaneous, gastrointestinal. Abstract This study aimed to investigate the risk factors associated with intracranial involvement in COVID-19-associated mucormycosis (CAM) and develop a nomogram model for predicting of involvement, specific focus on perineural spread. An ambispective analysis was conducted 275 CAM patients who received comprehensive treatment. Univariable multivariable logistic regression analyses were performed identify independent factors, created based results analysis. The performance evaluated using receiver operating characteristic (ROC) curve, discriminatory capacity assessed area under curve (AUC). model's calibration through Hosmer Lemeshow test. In results, revealed that age (OR: 1.23, 95% CI 1.06–3.79), HbA1c 7.168, 1.724–25.788), spread 6.3, 1.281–19.874), disease stage CAM. developed demonstrated good discriminative an AUC 0.821 (95% 0.713–0.909) as indicated by ROC curve. showed well-calibrated, test yielded P-value 0.992, indicating fit model. conclusion, this found particularly exhibits spread, which is predictive factor involvement. Introducción: La mucormicosis es una infección generada por hongos aerobios muy agresiva que pude involucrar vía aérea superior y extenderse a estructuras adyacentes. Objetivo: Analizar la asociación entre el deceso exenteración ocular en pacientes con rino-orbito-cerebral. Método: Se analizaron los aspectos clínicos desenlace de empleó estadística descriptiva, se Ji cuadrado para asociación, riesgo utilizó odd ratio, considerando como significativo un valor p≤0.05. Resultados: De 45, 11 no cumplieron criterios inclusión, encontraron asociaciones sexo, diabetes otras comorbilidades, estadios MROC deceso, COVID-19, ni hemoglobina glucosilada (HbA1c). Sin embargo, lisis ósea tomografía (p=0.001), tipo procedimiento quirúrgico (p=0.002), (p=0.001) asociaron significativamente deceso. diferencias niveles proteína C reactiva (PCR) (p=0.035), velocidad sedimentación globular (VSG) (p=0.056), recuento absoluto neutrófilos (p=0.000) muerte. Discusión: decisión ocular, aspecto complejo MROC, existen autores quienes señalan impacto psicológico proporción sustancial pacientes. COVID-19 causes an immunosuppressive condition and increases the risk of secondary infections, including fungal diseases such as mucormycosis (MM), so doctors should be aware possibility MM development in patients. Since no specific complaints or clinical symptoms exist disease is rare, especially developed countries with a moderate climate, it often impossible to diagnose at initial examination. Therefore, detection pathogen (mycelium zygomycetes) plays key role diagnosis. Over 70 % patients experience eye damages. According literature, most common manifestations thereof are blepharoptosis, ophthalmoplegia, exophthalmos, injection conjunctival chemosis. Quite often, have pains, orbital cellulitis phlegmon orbit, accompanied by decrease vision. For effective treatment this severe infectious complication, which takes form deep aggressive mycosis, interdisciplinary approach necessary. The complex make use antimycotic agents symptomatic therapy, and, essentially, surgical rehabilitation infection foci removing affected necrotic tissues. prognosis always very serious.

Abstract The outbreak of COVID-19 associated rhino-orbital-cerebral mucormycosis (CA-ROCM) through the latest waves pandemic in several countries gathered attention to this rare fungal infection. This prospective observational study aims evaluate demographic and medical profile, clinical signs symptoms (with a special focus on ophthalmic findings), imaging features, outcome CA-ROCM patients. collaborative Associated Rhino-Orbital-Cerebral Mucormycosis Iran (CA-ROCM-IR) has been conducted 8 tertiary referral ophthalmology centers among different provinces during fifth wave pandemic. All patients were subjected complete history taking comprehensive ophthalmological examination, underwent standard accepted treatment strategy based disease stage. present report is preliminary results project study. Two hundred seventy-four patients, including 150 males (54.7%), who had mean age 56.8 ± 12.44 years, enrolled. Diabetes mellitus was most prevalent (82.8%) Supplemental oxygen administration corticosteroid use due large proportion (68.2%, 73.7% respectively). common primary complaints facial pain (47.4%), swelling (38.3%), nasal discharge (32.5%). Ptosis (58%), periorbital (46%), congestion (40.5%) A majority (80.3%) diagnosed at stage 3 or higher ROCM rate ocular involvement whom 92.3%. affected paranasal sinus ethmoid (75.2%), followed by maxillary (70.8%). total 30 (10.9%) expired before from hospitals. Background: The coronavirus disease 2019 (COVID-19) pandemic was associated with an increased incidence of mucormycosis globally. However, the clinical pattern, epidemiologic features and risk factors for adverse outcomes are not well established. Methods: We performed a retrospective analysis data from patients hospitalized proven between April 2021 August 2021. Patients were managed multi-disciplinary approach involving medical, surgical, comorbidity treatment. presentation, management details, complications outcomes, including mortality, reviewed records. Results: mean age presentation 53.7 (± 11.8) years, 88 (84.6%) men. Of 104 cases COVID-19-associated mucormycosis, 97 (93.27%) had diabetes, 80.8% haemoglobin A1C (HbA1c) ≥6.4% at diagnosis. Seventy percent diabetes experienced steroid-induced hyperglycaemia during Even appropriate treatment, 17 (16.35%) died. High HbA1c creatinine levels, presence chronic kidney (CKD), need intensive care unit admission, orbital evisceration high mortality on multivariate logistic regression analysis. Cox revealed that overall by factor 12% each 1 percentage point increase in (hazard ratio 1.12; 95% confidence interval 0.95–1.31). even higher when CKD 1.82; 0.24–14.00). Conclusion: CKD, aggressive requiring predictors mucormycosis. these should be more actively to reduce morbidity mortality.
