Purpose of review The prevalence neurological complications among people with HIV (PWH) is expected to increase as PWH live longer due increased access antiretroviral treatment (ART). This provides updates the understanding sequelae, including neurocognitive impairment, neuropathy, opportunistic infections, and others, which are crucial for improving care outcomes PWH. Recent findings literature highlights several key themes: pathophysiology HIV-related neuronal damage involving proteins (gp120, Nef) neuroinflammation; role aging in exacerbating complications; high HIV-associated disorders (HAND) Alzheimer's disease-related dementias (AD/ADRD) PWH; importance screening tools like IHDS MoCA; identification biomarkers neuroimaging techniques early detection monitoring HAND. Summary highlight need comprehensive healthcare strategies manage PWH, targeted interventions high-risk groups, improved diagnostic tools, tailored treatments. It important clinicians researchers develop effective approaches mitigate impact on brain health improve quality life
