The brain and spinal cord, which constitute the central nervous system, were historically considered immune-privileged sites, as it was believed they lacked an equivalent to systemic lymphatic system. However, in 2013, a pathway facilitating clearance of waste products through parenchyma via perivascular space proposed, garnering attention 'glymphatic system'. The glymphatic pathway plays a crucial role in the clearance of metabolic byproducts and solutes from cerebral tissue. Dysfunction has been associated with various neurological disorders, including ischemic stroke. Diffusion tensor imaging (DTI) derived Analysis aLong Perivascular Space (ALPS) have emerged as promising tools for evaluating function. This review aims to summarize current evidence on use DTI-derived ALPS measures assessing dysfunction stroke patients, explore their potential implications diagnosis, prognostication, treatment monitoring this patient population.

Abstract Objective Diffusion-tensor imaging (DTI) and diffusion-weighted (DWI) along perivascular spaces (ALPS) index have been proposed as noninvasive techniques to indirectly evaluate the glymphatic system function. However, these are sensitive examination parameters, limiting inter-study comparability. The definition of region interest (ROI) has identified primary weakness ALPS method. Therefore, we aimed determine which ROI characteristics would best promote consistent analysis. Methods We examined 13 healthy volunteers using DTI DWI calculate index, compared determined correlations among 11 different configurations, tested inter-method reliability. Results found significant differences between configurations in calculation. Considering reliability, a squared is most suitable. ICC showed good-to-excellent agreement (mean = 0.83). did not find differences. Conclusion It important standardize for

This study seeks to determine if patients with high-grade glioma (HGG) demonstrate glymphatic system (GS) impairments using Diffusion Tensor Imaging Along Perivascular Spaces (DTI-ALPS). Additionally, it aims examine the factors affecting GS performance and their implications for HGG prognosis. The enrolled fifty alongside age- sex-matched healthy individuals. Each participant underwent diffusion tensor imaging a Philips 3.0T MRI scanner assess compute ALPS index within perivascular spaces. The glymphatic system, a vital brain perivascular network for waste clearance, hinges on the functionality of aquaporin 4 (AQP4) water channel. Alarmingly, AQP4 single nucleotide polymorphisms (SNPs) are linked to impaired or glymphopathy, which contributes sleep disturbances and various age-related neurodegenerative diseases. Despite critical role glymphopathy in cerebral small vessel disease (CSVD) – silent precursor disorders their interplay remains underexplored. CSVD is major cause stroke dementia, yet its pathogenesis not fully understood. Emerging evidence implicates as pivotal factors CSVD, exacerbating condition by hindering removal compromising blood-brain barrier (BBB) integrity. Advanced imaging techniques promise enhance diagnosis monitoring, while lifestyle modifications personalised medicine present promising treatment avenues. This narrative review underscores need multidisciplinary approach understanding CSVD. By exploring roles, emphasising necessity longitudinal studies, discussing potential therapeutic interventions, this paper aims pave way new research directions management.
