Abstract Introduction: This study aimed to investigate the association and causality between central obesity bone mineral density (BMD). Methods We utilized linkage disequilibrium score regression (LDSC) Mendelian randomization (MR) assess genetic correlations causal relationships waist circumference adjusted for BMI (WCadjBMI) total body less head BMD (TB-BMD). Additionally, a cross-sectional analysis of 7,452 participants evaluated relationship A shape index (ABSI) TB-BMD using weighted multivariable linear smooth curve fitting. Results LDSC MR confirmed negative WCadjBMI (β=-0.16; 95% CI: -0.26, -0.07). The indicated that an increase 0.01 ABSI corresponded decrease 0.035 (g/cm2), with this effect being particularly pronounced in males older adults. An inflection point was identified at = 0.076: below threshold, positively correlated pelvis BMD, whereas above it, became negative. Conclusions Central is significantly negatively related BMD. Maintaining within 0.058–0.078 crucial individuals mass accrual (20–30 years) stabilization (30–45 periods. In contrast, managing people experiencing early loss (45–60 presents greater complexity warrants further investigation.

We read with great interest the article by Ding et al. (1) entitled "Negative association between Body Roundness Index and bone mineral density: insights from NHANES." In this study, authors explored relationship body roundness index (BRI) total density (BMD) in U.S. adults. BRI, a newer anthropometric measure, has been shown to more comprehensively reflect visceral fat distribution compared other traditional indices. With weight is not part of equation, waist circumference measured along height plugged into formula estimate adipose tissue. The constructed three weighted multivariate regression models explore BRI BMD: Model 1 was unadjusted, 2 adjusted for key demographic variables, 3 all covariates. findings demonstrate notable inverse BMD, thus concluded that higher associated lower BMD potentially greater risk developing osteoporosis, advocating use as valuable marker early intervention. commend their rigorous study design analytical efforts; however, we would like raise several concerns offer suggestions further consideration help improve investigations. adverse effects inflammation on bones. detailed discussion 57 potential mechanisms through which accumulation promotes osteoporosis. 58The suggest using clinically assessing osteoporosis 60 achieving However, diagnostic ability evaluated 61 study. encourage perform receiver operating characteristic curve analysis compare 62 predictive obesity assessment indicators (not limited BMI) 63 novel measures, such Weight-adjusted Waist (6), A Shape (7), 64Abdominal Volume (8), Visceral Adiposity (9), Lipid Accumulation Product 65 (10). Despite accumulating evidence on central obesity and osteoporosis, the role of a body shape index (ABSI), nonlinear quantifying via mass (BMI), waist circumference (WC), height, remains controversial underexplored. Although recent meta-analyses suggest may modulate fracture risk bidirectionally, no research has comprehensively compared ABSI with traditional adiposity metrics, such as BMI, WC, waist-to-height ratio (WHtR), to predict site-specific changes in bone mineral density (BMD) across anatomical regions. This study utilized National Health Nutrition Examination Survey (NHANES) data from 2005 2018, involving 12,421 participants. was computed using formula: = WC/(BMI²/³ × Height¹/²). BMD assessed at four sites-the total femur (TF), femoral neck (FN), trochanter (TR), intertrochanter (IN) regions-via dual-energy X-ray absorptiometry (DXA). The association between analyzed multiple regression models generalized additive model (GAM). To compare ABSI's predictive efficacy conventional indices, analyses juxtaposed against WHtR assessing correlations BMD. After full covariate adjustment, significant negative observed regions (P< 0.01). Smoothed curve fitting revealed relationship threshold effect among middle-aged older individuals. Additionally, an inverted J-shaped all Meanwhile, showed associations sites (β -0.27 -0.31, p-trend< 0.000001), whereas exhibited positive (WHtR showing strongest effect: β 0.41-0.69). highlights ability detect adiposity-related loss obscured by metrics. robust inverse -0.31), persisting analyses, establish it novel biomarker skeletal fragility. This study investigates the association between blood Mn and bone mineral density (BMD), focusing on mediating role of sex steroids, using data from 8617 participants in National Health Nutrition Examination Survey (NHANES) 2013–2018. Weighted multiple linear regression models were used to examine total BMD, mediation analyses explored roles testosterone (TT), estradiol (E2), hormone-binding globulin (SHBG) Mn-BMD relationship, stratified by menopausal status. Blood was negatively associated with BMD both sexes, a pronounced effect postmenopausal women. SHBG mediated 37.16% men, whereas no effects found E2 exhibited significant indirect effect, suggesting that reduced levels may amplify Mn’s BMD. These findings indicate exposure is decreased potentially through alterations highlighting importance considering hormone status when evaluating impact
