Purpose of review The use androgenic anabolic steroids (AAS) is rising, particularly among recreational athletes. This addresses growing concerns about the underrecognized cardiovascular and multiorgan consequences chronic AAS exposure with a focus on noncompetitive populations. Recent findings It well documented that enhances muscle mass strength, but at cost multisystem toxicity including endocrine disruption, hepatotoxicity, mood disorders. Emerging evidence highlights profound impact use, elevated blood pressure, adverse lipid profiles, accelerated atherosclerosis, subclinical cardiomyopathy, increased risk myocardial infarction sudden cardiac death. Structural functional abnormalities such as left ventricular hypertrophy, dysfunction, arterial stiffness have been reported, some changes persisting after cessation. Summary carries risks for remodeling atherogenesis. Clinicians caring athletes using should recognize patterns provide stratification, monitoring, tapering strategies. Future research prioritize long-term outcomes, sex-specific effects, multidisciplinary approaches to care.
