Abstract Neuropathic pain (NP) is a common chronic with heterogeneous clinical features, and consequent lowering of quality life. Currently, although conventional chemical drugs can effectively manage NP symptoms in the short term, their long‐term efficacy limited, they come significant side effects. In this regard, traditional Chinese medicine (TCM) provides promising avenue for treating NP. Numerous pharmacological studies have substantiated effectiveness TCM multiple targets mechanisms. We aimed to outline characteristics TCM, including compound prescriptions, single herbs, active ingredients, physical therapy, treatment discussed by analyzing pathogenesis Various databases, such as PubMed, Web Science, China National Knowledge Infrastructure, Wanfang database, were searched. focused on recent research progress TCM. Finally, we proposed future challenges emerging trends demonstrates treatment, employing multi‐mechanisms. Drawing from theory syndrome differentiation, four types dialectical treatments prescriptions introduced: promoting blood circulation removing stasis; promote Qi flow; warming Yang benefiting Qi; soothing liver regulating Qi. Meanwhile, 33 herbs 25 ingredients included. addition, therapy (e.g., acupuncture, massage, acupoint injection, fumigation) also showed good treatment. particularly through use holds bright prospects owing its diverse holistic Nonetheless, multi‐targets may result possible disadvantages mechanisms need further evaluation. Here, provide an overview via based potential therapeutic mechanisms, thus providing reference studies.

Background Neuropathic pain presents a significant clinical challenge, with spinal cord epigenetic mechanisms playing critical role in its development. This study investigated the impact of nerve injury on Barrier-to-Autointegration Factor (BAF) rat dorsal horn. Methods Adult Sprague-Dawley rats underwent ligation (SNL) to model neuropathic pain. Pain behaviors were assessed using von Frey and burrow tests. Biochemical analyses measured mRNA protein expression Results SNL elevated BAF levels, which interacts LEM domain–containing 2 (LEMD2), activating histone-modifying enzyme EZH2. adds gene-silencing mark, H3K27me3, promoter region Oprm1 gene, encodes mu-opioid receptor. Consequently, receptor is decreased, potentially contributing Using gene knockdown techniques reduce expression, we reversed changes LEMD2, EZH2, expressions induced by attenuated mechanical allodynia. Additionally, knocking down LEMD2 disrupted binding promoter, without affecting levels. Inhibiting EZH2 also signaling altering Glutamate activated pathways via pNR2B receptors, NR2B blockade this effect. Conclusion These findings suggest that receptors may activate BAF, enhance EZH2-mediated H3K27me3 at after injury. Targeting pathway offer novel strategies inhibit

Intravesical botulinum toxin A (BoNT-A) injections are included in the interstitial cystitis/bladder pain syndrome (IC/BPS) treatment guidelines. However, IC phenotype suitable for with BoNT-A has not been clarified. Therefore, we identified factors influencing outcomes intravesical patients non-Hunner IC/BPS (NHIC). This retrospective study NHIC who underwent 100 U over past two decades. Six months after treatment, were assessed using Global Response Assessment (GRA). Outcome endpoints GRA, clinical symptoms, urodynamic parameters, urine biomarkers, and identification of contributing to satisfactory outcomes. The 220 (42 men, 178 women). group (n = 96, 44%) had significantly higher severity scores symptoms index, larger maximum bladder capacity (MBC), lower 8-isoprostane levels at baseline. Logistic regression revealed that MBC (≥760 mL) predominance associated injection. Subjective parameters improved pain-predominant Thus, or pelvic more likely experience following injections.

Inflammation during the perioperative period of joint arthroplasty is a critical aspect patient outcomes, influencing both pathophysiology pain and healing process. This narrative review comprehensively evaluates roles specific cytokines inflammatory biomarkers in this context their implications for management. Inflammatory responses are initiated propagated by cytokines, which pivotal development acute chronic postoperative pain. Pro-inflammatory play essential up-regulating response, which, if not adequately controlled, leads to sustained impaired tissue healing. Anti-inflammatory work dampen promote resolution. Our discussion extends genetic molecular influences on cytokine production, influence perception recovery rates post-surgery. Furthermore, role PAI-1 modulating inflammation through its impact fibrinolytic system highlights potential as therapeutic target. The modulation these various analgesic anesthetic techniques, including fascia iliac compartment block, demonstrates significant reduction markers, thus underscoring importance targeted strategies. analysis suggests that nuanced understanding interplay between pro-inflammatory anti-inflammatory required.
