Pharmacokinetic and pharmacodynamic alterations in older people: what we know so far

Healthcare professionals face increasing challenges when managing older patients, a group characterized by significant interindividual variability in comorbidity patterns, homeostatic capacity, frailty status, cognitive function, and life expectancy. Complex therapeutic decisions may increase the risk of inappropriate polypharmacy, drug-drug, drug-disease interactions context age-associated pharmacokinetic pharmacodynamic alterations, with consequent drug accumulation toxicity. This state-of-the-art narrative review article summarizes critically appraises results original research studies reviews published PubMed, Scopus, Web Science, from inception to 9 April 2025, on changes critical organs systems relevant alterations. It also discusses emerging role gut microbiota influencing such alterations potential utility machine learning techniques identifying new signals efficacy toxicity patients. The available knowledge regarding specific applies limited number drugs, some which are not frequently prescribed contemporary practice. Future investigating wider range drugs their patterns use will likely enhance minimize patient population.
