Smartwatch-based functional assessments for upper extremity movement are a promising tool detailed and serial assessment during stroke rehabilitation, but their clinical application remains challenging. In this study, nine patients with hemiparesis due to participated in occupational therapy sessions using virtual reality-based rehabilitation devices. An Action Research Arm Test (ARAT) was performed at baseline after intervention, wrist smartwatch sensors recording motion data. We extracted acceleration gyro sensor data from smartwatches calculated the average segment size (MSS) as measure of smoothness. Among included patients, four participants completed all 10 follow-up evaluation. The MSSs x, y, z directions were significantly correlated ARAT scores across task domains. For angular motion, MSS gross (domain 4) showed strong correlations scores: roll (rs = 0.735, p 0.004), pitch 0.715, 0.009), yaw 0.704, 0.007). At follow-ups, most considerable increase roll, pitch, angles measured domain 4, alongside improvements scores. Our findings support feasibility commercial smartwatch-based parameters evaluations highlight potential assessments.
