Introduction: The USA has the poorest health statistics of any high-income country. Political polarization risen dramatically; newer safety net programs (the Affordable Care Act [ACA]) are unevenly provided because many Republican-leaning states refused expanded Federal coverage. Democratic have reduced physician leadership medicine. Both been deleterious. Here, we investigated associations among four key measures two which directly impact pregnancy outcomes and that affect all patients by percentage each state voted for Republican versus candidate in 2020 presidential election. Methods: For state, used public, non-partisan databases to assess incidence COVID, maternal, infant mortality per 100,000 population average life expectancy. Correlations these outcome variables vote were calculated (r), contextualized measuring with related including COVID vaccination rates, access medical care, incidences heart disease, obesity, diabetes, gunshot deaths, automotive fatalities. Results: mortality, maternal expectancy highly correlated (“red”) state. If “red” had rates equivalent Democratic-leaning (“blue”) states, 72,000 deaths could avoided. Overall, lower metrics, higher comorbidities. Conclusion: percent was strongly associated, but not whole answer, worse multiple public various ACA improved patient care also led “maternity deserts” disproportionately rural areas states. Translating insurance coverage into requires further analysis will require multi-pronged approaches expanding incentivizing quality care.
