Importance Esketamine has been found to reduce the incidence of postpartum depression (PPD) in randomized clinical trials. However, current evidence from trials does not reflect esketamine’s efficacy settings. Objective To assess intraoperative esketamine administration for preventing PPD among women who underwent cesarean delivery. Design, Setting, and Participants This trial was conducted at The First Affiliated Hospital Chongqing Medical University Chongqing, China, March 2023 February 2024. Pregnant patients admitted delivery were included, while those with intellectual dysfunction or contraindications excluded. All participants assigned randomly either group control a 1:1 ratio. Data analysis based on intention-to-treat principle. Interventions Patients received an infusion 0.25 mg/kg 20 mL saline over minutes, whereas minutes. Postpartum depression (PPD) significantly impacts women's mental health and social functioning, yet effective therapies remain limited. This study investigates the preventive effects of music therapy on PPD-like behaviors underlying neurobiological mechanisms in a mouse model subjected to ovarian hormone withdrawal (HW). Mice exposed daily sessions exhibited markedly reduced depression- anxiety-like behaviors, as evidenced by enhanced performance behavioral tests such open field test (OFT), forced swim (FST), elevated plus maze (EPM), sucrose preference (SPT), novelty-suppressed feeding (NSF) test, tail suspension (TST). Furthermore, normalized oxidative stress indicators (NO, MDA, SOD, CAT, GSH-Px, T-AOC, ATP, glutamate) serum, hippocampus, prefrontal cortex. Additionally, exposure levels proinflammatory factors (IL-6, IL-1β, iNOS, TNF-α, TGF-β) activation microglia astrocytes these brain regions. Notably, preserved neuronal integrity, promoted neurogenesis, maintained synaptic plasticity, restoration dendritic spines. Transcriptome sequencing identified differential gene expression pathways related inflammation, stress. These findings suggest that prevents PPD modulating stress, providing robust preclinical evidence for its potential natural intervention PPD. underscores need further clinical research validate therapeutic efficacy preventing humans, highlighting promise non-invasive accessible treatment modality.

Randomized controlled trials and meta-analyses efficacy esketamine in preventing depression after cessarean section. However, data included these analyses were derived from elective sections differed dose timing administration. dissociative anesthetic dose-dependent risk inducing psychotic symptoms, including hallucinations. In setting section, should be administered caution only if potential benefits outweigh risks.
