Currently, despite continued issues with durability ( 1), biological prosthetic valves are increasingly chosen over mechanical for surgical aortic valve replacement (SAVR) in adult patients of all ages, at least Western countries. For younger patients, this choice means assuming the risks associated a redo SAVR or valve-in-valve procedure. To assess use vs. prostheses relative to patient's age and implant time large population extracted from French National Database EPICARD.Patients EPICARD undergoing 2007 2022 were included 22 participating public private centers represent balanced representation centre sizes geographical discrepancies. Patients pathology aorta (aneurysm dissection) requiring vascular prosthesis excluded. Comparisons made amongst centers, choice, date range, patient age.We considered 101,070 valvular heart disease 72,375 (mean 71.4 ± 12.2 years). We observed ratio (MBPR) 0.14 overall population. Before 50 years old (y-o), MBPR was >1.3 (p < 0.001) while above 60 years-old received principally 0.0001). INTRODUCTION: Identification of risk factors that cause a high probability an unfavorable outcome in the postoperative period is urgent problem. The creation national databases (registries) makes it possible to cover certain patient population by identifying its predictors. Existing registries differ criteria for inclusion study, characteristics populations studied, and there often no common view on classification outcomes. OBJECTIVE: Creation Russian calculator complications mortality. MATERIALS AND METHODS: Two-level observational retrospective-prospective study. Setting: National multicenter study surgical inpatients. Patients: Adult patients undergoing elective emergency surgery. Types interventions: obstetrics, gynecology, mammary gland, urology, endocrine surgery, maxillofacial orthopedics, traumatology, abdominal liver biliary tract, thoracic vascular neurosurgery, cardiac other areas. RESULTS: design was registered ClinicalTrials.gov database, organized Federation Anesthesiologists Reanimatologists Russia. Primary (30-day mortality, 30-day complications) secondary (hospital hospital complications, length stay ICU, stay, multiple organ failure (2 or more points SOFA scale), 90-day post intensive care syndrome, readmission, 1-year mortality) outcomes; six primary twelve target points; inclusion, non-inclusion, exclusion. required sample size statistical analysis are described. planned ensure power determined be 60,800 observations surgery 20,000 duration 2024–2028. CONCLUSIONS: has important scientific medical-social significance; mortality will developed. In future, developed can become basis making medical decisions.

Noteworthy in Cardiothoracic Surgery 2023 summarizes a few of the most high-impact trials and provocative trends cardiothoracic surgery transplantation this past year. Transplantation using organs procured from donation after circulatory death (DCD) continues to increase, American Society Transplant Surgeons released recommendations on best practices 2023. We review summary data impact DCD heart lung transplantation. There has been increased interest extracorporeal life support (ECLS), particularly COVID-19 pandemic, we results highly discussed ECLS-SHOCK trial, which randomized patients cardiogenic shock with planned revascularization ECLS vs usual care. With improving survival outcomes complex aortic surgery, there is need for higher-quality evidence guide cooling cerebral perfusion strategies may optimize cognitive these patients. short-term GOT ICE trial (Cognitive Effects Body Temperature During Hypothermic Circulatory Arrest), multicenter, controlled three different nadir temperatures, evaluating cognition associated changes functional magnetic resonance imaging. Finally, both Thoracic (STS) College Cardiology, Heart Association, Chest Physicians Rhythm (ACC/AHA/ACCP/HRS) updated atrial fibrillation guidelines 2023, surgically relevant updates behind them.
