The tryptophan (TRP)-kynurenine (KYN) pathway is involved in the pathogenesis of schizophrenia. This study aimed to investigate levels TRP-KYN metabolites serum and urine patients with first-episode schizophrenia (FES) their association clinical manifestations. included 38 drug-naive FES 43 healthy controls (HCs). Clinical symptoms were evaluated using Positive Negative Syndrome Scale (PANSS). Levels quantified. Patients showed significantly higher quinolinic acid/kynurenic acid (QUIN/KYNA) ratio KYN/TRP compared HCs, while neuroprotective metabolites, including KYNA, xanthurenic (XA), picolinic (PIC) levels, reduced, along a decreased PIC/QUIN (p < 0.05). KYNA/KYN was negatively correlated PANSS general psychopathology scores (r = -0.35, p 0.04) total 0.046). exhibited dysregulation peripheral pathway, characterized by an increased neurotoxic-to-neuroprotective QUIN/KYNA reduced metabolites. shift towards neurotoxic product generation suggests that could play role pathophysiology
