Objectives This study aimed to investigate the chemico-pharmacological properties of leaves methanolic extract (ME) Zanthoxylum rhetsa (Roxb.) and its various solvent fractions, focusing on antioxidant, cytotoxic, thrombolytic, anti-inflammatory, analgesic, hypoglycaemic, antidiarrheal effects. Methods Phytochemicals in ME were identified characterized using gas chromatography mass spectrometry (GC-MS). Antioxidant activity was measured via DPPH (2, 2-diphenyl-1-picrylhydrazyl) scavenging assay measuring total phenolic content (TPC). Anti-inflammatory, cytotoxic effects evaluated membrane stabilization, clot lysis, brine shrimp bioassays, respectively. In vivo evaluations antidiarrheal, analgesic activities (200, 400 600 mg/kg bw) conducted mice model. Results A 65 phytoconstituents ME, where 13-docosenamide (Z) (17.78%) phytol (10.64%) most abundant. The aqueous soluble fraction (AQSF) exhibited highest TPC (181.875 mg GAE/gm extractive) antioxidant efficacy (IC 50 = 4.78 μg/mL). AQSF also promising antithrombotic properties, showing over 88% lysis capacity. Furthermore, chloroform (CSF) displayed 93.2% haemolysis inhibition under heat-induced conditions, while 46.8% hypotonic conditions. All tested three doses showed dose-dependent statistically significant (p < 0.05) efficacy, only 200 bw hypoglycaemic acute oral toxicity test, median lethal dose greater than 4000 bw. Conclusions Z. leaf extractives demonstrated antithrombotic, effects, supporting traditional uses plant species. Further extensive investigation, particularly fraction, is recommended identify responsible lead compounds.
