Abstract Background Several pioneering studies investigated deep brain stimulation (DBS) in treatment-refractory anorexia nervosa (AN) patients, but overall effects remain yet unclear. Aim of this study was to obtain estimates efficacy DBS AN-patients using meta-analysis. Methods We searched three electronic databases until 1st November 2021, terms related and AN. included trials that the clinical AN-patients. obtained data including psychiatric comorbidities, medication use, target, duration. Primary outcome Body Mass Index (BMI), secondary quality life, severity symptoms, eating disorder, obsessive-compulsive, depressive, anxiety symptoms. assessed risk bias ROBINS-I tool. Results Four were for meta-analysis, with a total 56 patients Follow-up ranged from 6–24 months. Random meta-analysis showed significant increase BMI following DBS, large effect size (Hedges’s g = 1 ∙ 13; 95% CI 0 80 46; Z -value 6 75; P < 001), without heterogeneity ( I 2 00, 901). also life 86; 44 28; 4 01, 001). Furthermore, decreased symptoms 89; 57 21; 5 47; 001, 29, 371). Anorexia nervosa and obesity are common appetite disorders, which may be life threatening if not treated often coincide with psychiatric disorders. We sought to investigate whether deep brain stimulation (DBS) of specific regions within the could aid in treatment these This review aims organize literature regarding feasibility DBS via clinical outcomes synthesize data on patient demographics electrode parameters for future optimization.PubMed, Scopus, Web Science databases were all queried 7 June 2022 identify studies reporting effect either anorexia or obesity. included involving 1) DBS, 2) obesity, 3) body mass index (BMI) as primary outcome variable. Case reports, retrospective cohort studies, randomized controlled trials eligible inclusion. Exclusion articles was based following criteria: meta-analyses systematic reviews describes diseases other than only Screening 999 returned by an initial search yielded 23 inclusion further extraction. Qualitative assessment subsequently conducted accordance Newcastle-Ottawa Scale criteria. We (17 anorexia, 5 obesity) that met our exclusion criteria, 8 case 13 series, 1 case-control study. Our variables interest location change BMI after intervention, parameters, comorbidities. A total 131 patients analyzed, 118 those belonging cohort. For we found most place subcallosal cingulate followed nucleus accumbens, resulting overall increase 24.82% over span a mean 17.1 months. Psychiatric comorbidities (major depressive disorder, obsessive-compulsive anxiety) lateral hypothalamus small decrease 3.97% 17.2 Data insufficient this report additional calculate duration from diagnosis treatment.DBS seems promising solution addressing treatment-refractory but prospective still needed confirm same usefulness Primary limitations apparent lack well dearth assessing efficacy compared control treatments. Abstract Background Anorexia nervosa has one of the highest mortality rates all mental illnesses. For those who survive, less than 70% fully recover, with many going on to develop a more severe and enduring phenotype. Empirical evidence and clinical observations suggest a strong -yet under acknowledged-link between anorexia nervosa (AN) non-suicidal self-injurious behavior (NSSI). By reviewing the literature on psychopathology neurobiology of AN NSSI, we shed light their relationship. Both NSSI are characterized by disturbances in affect regulation, dysregulation reward circuitry opioid system. formulating reward-centered hypothesis, explain overlap NSSI. We propose three approaches understanding relationship which integrate from perspective self-destructiveness: (1) nosographical approach, (2) research domain (RDoC) approach (3) network analysis approach. These will enhance our knowledge underlying neurobiological substrates may provide groundwork for development new treatment options disorders self-destructiveness, like In conclusion, hypothesize that self-destructiveness is new, DSM-5-transcending concept or psychopathological entity reward-driven, both could be conceptualized as self-destructiveness.

Psychiatric disorders are among the leading contributors to global disease burden and disability. A significant portion of patients with psychiatric remain treatment-refractory best available therapy. With insights from neurocircuitry extensive experience neuromodulation deep brain stimulation (DBS) in movement disorders, DBS is increasingly being considered modulate neural network disorders. Currently, obsessive-compulsive disorder (OCD) only U.S. FDA (United States Food Drug Administration) approved indication for Medically refractory depression, addiction, other explored neuromodulation. Studies evaluating promising but lack larger, controlled studies. This paper presents a brief review current state neurosurgical therapies OCD We also present MR-guided Focused Ultrasound (MRgFUS), novel form neuromodulation, which can target subcortical structures similar DBS, noninvasive fashion. Early experiences therapies, including MRgFUS encouraging disorders; however, they investigational. VNS options properly selected cases respectively.

Anorexia nervosa (AN) is a severe and life-threatening psychiatric disorder. Initial studies on deep brain stimulation (DBS) in severe, treatment-refractory AN have shown clinical effects. However, the working mechanisms of DBS remain largely unknown. Here, we used task-based functional MRI approach to understand pathophysiology AN.We performed four patients that participated pilot study efficacy, safety, effects targeted at ventral limb capsula interna (vALIC). Hypothalamic dysfunction may result in morbid obesity as a consequence of decreased energy expenditure, feelings satiety, and increased fat storage. In patients with hypothalamic dysfunction, neurobehavioral is also often present. Currently, no effective treatment has been found for (HO). We hypothesize that deep brain stimulation (DBS) be an aiming to treat HO well the dysfunction.A systematic search was conducted PubMed, EMBASE Cochrane Library databases studies published until May 2022 reporting on DBS HO.Three met predetermined inclusion criteria, total six treated HO, which five Prader-Willi syndrome (PWS) one patient after craniopharyngioma (CP). Targets included lateral area (LHA) nucleus accumbens (NAcc). PWS, LHA-DBS associated mean increase Body Mass Index (BMI) (+5.8%), change hormonal levels, results blood workup, sleep, or neuropsychological evaluation. CP, NAcc-DBS decrease BMI (-8.7%) subjective mental health, willingness act, feeling appetite. No objective measurements function were reported. severe adverse events reported these cases. Mild moderate hypomanic symptoms infection. All described follow-up period (n = 5) able sustain at least 6 months few interruptions. There limited research HO. The effectiveness differed across evidence limited. Although there potential severe-refractory condition more needed target selection evaluation effectiveness.

Ubiquitous technology, big data, more efficient electronic health records, and predictive analytics are now at the core of smart healthcare systems supported by artificial intelligence. In present narrative review, we focus on sensing technologies for Anorexia Nervosa (AN). We employed a framework inspired Interpersonal Neurobiology Theory (IPNB), which posits that human experience is characterized flow energy information both within us (within our whole body), between (in connections have with others nature). line this framework, focused sensors designed to evaluate bodily processes (body such as implantable sensors, epidermal wearable portable sensors), social interaction (sociometric physical environment (indoor outdoor ambient sensors). There myriad man-made well nature-based plants can be used design deploy intelligent monitoring healthcare. conclusion, smarter AN help relieve burden professionals. However, there technical, ethical, environmental sustainability issues must considered prior implementing these systems.
