Postoperative cognitive dysfunction (POCD) is a common neurological complication in elderly patients after surgery and general anesthesia. The occurrence of POCD seriously affects the postoperative recovery patients, leads to prolonged hospital stay, reduced quality life, increased medical costs, even higher mortality. There no definite effective drug treatment for POCD. More evidence shows that perioperative non-pharmacological intervention can improve function reduce incidence Therefore, our studies summarize current interventions from aspects training, physical activity, transcutaneous electrical acupoint stimulation, noninvasive brain sleep improvement, music therapy, environment, multimodal combination Interventions, provide more data clinical application research.

Postoperative cognitive dysfunction (POCD) is a common complication after total knee arthroplasty (TKA), impacting recovery and quality of life. This study aims to investigate central sensitization (CS) as an independent risk factor for POCD improve preoperative screening postoperative interventions. A retrospective analysis was conducted on 142 TKA patients from January 2020 May 2024 across three hospitals. Data were collected at six time points: preoperatively (T0), intraoperatively (T1), postoperatively days 1 (T2), 3 (T3), 7 (T4), 30 (T5). Patients classified into CS (CSI ≥ 40) non-CS < groups according Central Sensitization Inventory (CSI) score. Cognitive function incidence assessed with the Mini-Mental State Examination (MMSE), Knee Injury Osteoarthritis Outcome Score (KOOS). Logistic regression used identified factors POCD. The overall T5 19.72%, significantly higher rate in group (30.91%) compared (12.64%) (p = 0.008). MMSE scores declined both T2 T3 T0 0.05), showing consistently lower than T2-T5 0.05). KOOS revealed that had worse pain life T0, T4, CS, cerebrovascular disease, intraoperative hemorrhage, associated significant following TKA, adversely affecting terms Prospective studies are warranted validate findings develop targeted

Abstract Photo‐biomodulation (PBM) with near‐infrared (NIR) laser emerges as a promising noninvasive therapy for enhancing cognitive function. Here the effects of PBM, equipped an 808 nm NIR laser, on function are investigated when used to irradiate hippocampi mice. irradiation once day 5 min per consecutive days significantly improves learning and memory performance naïve mice assessed Barnes Y maze tests. PBM also attenuates dysfunction following anesthesia surgery, lipopolysaccharide challenge, respectively. Cognitive enhancements associated increased neuronal oscillations enhanced theta–gamma coupling in hippocampal dentate gyrus (DG) during processing. High activation DG is evidenced by c‐Fos expression calcium signal intensity. The improvements high‐density cells together facilitating local energy deposition activation. NIR‐laser‐activated axonogenesis oxidative phosphorylation‐related genes single‐cell RNA sequencing were seen DG. These findings suggest that very likely holds great potential dysfunction.

To evaluate the efficacy of transcranial direct current stimulation (tDCS) in reducing incidence postoperative delirium (POD) elderly patients undergoing hip fracture surgery. This single-center, double-blind, randomized controlled trial will enroll 160 participants aged 65 years and older, scheduled for elective surgery under spinal anesthesia. Participants be randomly assigned to either active-tDCS group or sham-tDCS group. The receive two sessions: one pre-surgery post-surgery, with electrodes positioned over left dorsolateral prefrontal cortex right supraorbital area. Each session includes 15-second ramp-up phase at start, 20 min simulation 2 mA ramp-down end. sham procedures no actual delivered. Functional brain activity monitored before after each procedure assess changes cortical activation connectivity using functional near-infrared spectroscopy (fNIRS). primary outcome measure POD, assessed 3-Min Diagnostic Interview Confusion Assessment Method (3D-CAM). Secondary outcomes include severity delirium, pain, anxiety, depression, cognitive function, sleep quality. was registered ClinicalTrials.gov (NCT06678529) on Oct 22, 2024.

To explore the relationship between systemic inflammation response index (SIRI) and postoperative delirium (POD) in older patients with hip arthroplasty surgery.Older who underwent elective surgery were included this retrospective study. SIRI, neutrophil-to-lymphocyte ratio (NLR) platelet-to-lymphocyte (PLR) collected from blood routine examination at admission. Binary logistic regression analysis was performed to evaluate association SIRI levels POD analyzed.Ultimately, 116 met inclusion criteria assessed. Thirty-four (29%) of diagnosed defined as group, rest consisted Non-POD group. Compared non-POD patients, showed significantly higher (P < 0.001) NLR = 0.002) There no significance PLR two groups. independently associated occurrence multivariate [odds (OR) 3.34, 95% confidence interval (95% CI) 1.26-8.85, P 0.016]. Receiver operating characteristic curve indicated that an optimal cutoff value 0.987 predicted a sensitivity 88.2% specificity 74.4%, area under 0.82 CI, 0.74-0.90, 0.01).Preoperative are POD. Moreover, preoperative level is useful candidate biomarker identify after patients.
