The acronym ESKAPE includes six nosocomial pathogens that exhibit multidrug resistance and virulence: Enterococcus faecium, Staphylococcus aureus, Klebsiella pneumoniae, Acinetobacter baumannii, Pseudomonas aeruginosa Enterobacter spp. Persistent use of antibiotics has provoked the emergence resistant (MDR) extensively drug (XDR) bacteria, which render even most effective drugs ineffective. Extended spectrum β-lactamase (ESBL) carbapenemase producing Gram negative bacteria have emerged as an important therapeutic challenge. Development novel therapeutics to treat infections, especially those caused by is need hour. Alternative therapies such in combination or with adjuvants, bacteriophages, antimicrobial peptides, nanoparticles photodynamic light therapy are widely reported. Many reviews published till date describe these respect various agents used, their dosage details mechanism action against MDR but very few focused specifically on ESKAPE. objective this review alternative reported advantages limitations, potential application vivo status clinical trials. further highlights importance a combinatorial approach, wherein two more used order overcome individual additional studies warranted, before translating them into practice. These advances could possibly give alternate solution extend lifetime current antimicrobials.

Abstract Bacteriophages are not forgotten viruses anymore: scientists and practitioners seek to understand phage pharmacokinetics in animals humans, investigating bacteriophages as therapeutics, nanocarriers or microbiome components. This review provides a comprehensive overview of factors that determine circulation, penetration, clearance, consequence applicability for medicine. It makes use experimental data collected by the community so far (PubMed 1924‐2016, including non‐English reports), combining elements critical systematic review. study covers ability enter system various routes administration, how (and if) may access tissues organs, finally what mechanisms courses clearance. The method was applied analyze (i) survival gut (gut transit) (ii) mammalian many administration routes. Aspects have yet been covered sufficient number reports mathematical analysis, well underlying trends, discussed form In spite extraordinary diversity possible applications, analysis revealed morphology, specificity, dose, presence sensitive bacteria characteristics treated individuals (age, taxonomy) affect bioavailability humans. However, once phages successfully body, they reach most central nervous system. cleared mainly immune system: innate immunity removes even when no specific response has developed.

Antibiotic resistance is a major public health challenge worldwide, whose implications for global might be devastating if novel antibacterial strategies are not quickly developed. As natural predators of bacteria, (bacterio)phages may play an essential role in escaping such dreadful future. The rising problem antibiotic has revived the interest phage therapy and important developments have been achieved over last years. But where do we stand today what can expect from future? This question set to answer this review. Here, scour outcomes human clinical trials case reports, address barriers that way using phages settings. We particularly potential hinder discuss future avenues explore full capacity therapy.

Phage therapy is increasingly put forward as a “new” potential tool in the fight against antibiotic resistant infections. During “Centennial Celebration of Bacteriophage Research” conference Tbilisi, Georgia on 26–29 June 2017, an international group phage researchers committed to elaborate expert opinion three contentious related issues that are hampering clinical progress field therapy. This paper explores and discusses bacterial resistance, training presence prophages production strains while reviewing relevant research findings experiences. Our purpose inform stakeholders such policy makers, officials competent authorities for medicines, producers, members pharmaceutical industry. brief also points out avenues future development it specifically addresses those overarching questions currently call attention whenever phages go into purification processes application.

The use of viruses infecting bacteria (bacteriophages or phages) to treat bacterial infections has been ongoing clinically for approximately 100 years. Despite that long history, the growing international crisis resistance standard antibiotics, abundant anecdotal evidence efficacy, and one successful modern clinical trial this phage therapy is not yet a mainstream approach in medicine.
