Perinatal women living with HIV face increased susceptibility to mental health challenges, including suicidal ideation (SI). This study aimed determine the prevalence of SI among perinatal and identify associated factors. A systematic search was done across PubMed, Embase, Scopus, ScienceDirect, Google Scholar. Data analysis executed using R software. Publication bias assessed via funnel plot Egger's test, while heterogeneity investigated I2 statistic. total 11 studies involving 4329 participants were included. The pooled 23.4% (95% CI: 16.3–32.4). Subgroup showed higher in postnatal (36.4%) than antenatal (27.8%), although this difference not statistically significant. Studies employing Edinburgh Postnatal Depression Scale reported a (38.9%). published between 2013–2017 (37.6%) compared those 2018–2022 (18.2%). Factors included depression during pregnancy or postpartum, unplanned pregnancy, intimate partner violence, undisclosed status, lack social support, recent diagnosis sexually transmitted infections other HIV. high emphasizes need for screening interventions. Mental assessments should be integrated into routine care.
