Many uncertainties still surround the possible harmful effect of fluoride exposure on cognitive neurodevelopment in children. The aim this systematic review and meta-analysis was to characterize relation through a dose-response approach, by comparing intelligence quotient (IQ) scores highest versus lowest category with random-effects model, within one-stage based cubic spline model. Out 1996 potentially relevant literature records, 33 studies were eligible for review, 30 which also suitable meta-analysis. summary mean difference IQ score, categories considering all types exposure, -4.68 (95% confidence interval-CI -6.45; -2.92), value -5.60 CI -7.76; -3.44) drinking water -3.84 -7.93; 0.24) urinary fluoride. Dose-response analysis showed substantially linear decrease increasing above 1 mg/L, -3.05 -4.06; -2.04) points per mg/L up 2 becoming steeper such level. A weaker -2.15 -4.48; 0.18) emerged 0.28 (approximately reflecting content 0.7 mg/L). inverse association between particularly strong at high risk bias, while no adverse only study judged low bias. Overall, most suggested an children's IQ, starting levels exposure. However, major role residual confounding could not be ruled out, thus indicating need additional prospective bias conclusively assess neurodevelopment.

Importance Previous meta-analyses suggest that fluoride exposure is adversely associated with children’s IQ scores. An individual’s total comes primarily from in drinking water, food, and beverages. Objective To perform a systematic review meta-analysis of epidemiological studies investigating scores prenatal or postnatal exposure. Data Sources BIOSIS, Embase, PsycInfo, PubMed, Scopus, Web Science, CNKI, Wanfang, searched through October 2023. Study Selection Studies reporting scores, exposure, effect sizes. Extraction Synthesis were extracted into the Health Assessment Workplace Collaborative system. quality was evaluated using OHAT risk-of-bias tool. Pooled standardized mean differences (SMDs) regression coefficients estimated random-effects models. Main Outcomes Measures Children’s Results Of 74 included (64 cross-sectional 10 cohort studies), most conducted China (n = 45); other locations Canada 3), Denmark 1), India 12), Iran 4), Mexico New Zealand Pakistan 2), Spain Taiwan 1). Fifty-two rated high risk bias 22 low bias. Sixty-four reported inverse associations between measures IQ. Analysis 59 group-level dental fluorosis, (47 bias, 12 bias; n 20 932 children) showed an association (pooled SMD, −0.45; 95% CI, −0.57 to −0.33; P < .001). Abstract Background Fluoride may be a developmental neurotoxicant at elevated exposures. We merged new data from prospective Odense Child Cohort (OCC) with results two previous birth cohort studies Mexico and Canada to characterize the dose–effect relationship in greater detail. Methods The OCC contributed 837 mother–child pairs total of >1500. measured creatinine-adjusted urine-fluoride concentrations maternal urine samples obtained during late pregnancy. IQ was determined age 7 years using an abbreviated version Wechsler Intelligence Scales for Children. Findings three cohorts were used calculate joint benchmark concentration (BMC) lower confidence limit (BMCL) after adjustment covariables. Results In OCC, varied between 0.08 3.04 mg/l (median 0.52 mg/l) but not significantly associated full-scale (β = 0.08; 95% interval −1.14 1.30 doubling exposure). No difference apparent boys girls. BMC 0.92 mg/l, BMCL 0.30 mg/l. analysis all showed statistically significant association IQ, 0.45 (BMCL, 0.28 mg/l), slightly higher than previously reported North American alone. Conclusions As reflects approximate threshold neurotoxicity, suggest that pregnant women children need protection against fluoride toxicity.
