Background: An increased risk of cognitive decline has been reported in patients with older age bipolar disorder (OABD); however, the underlying factors contributing to this association remain unclear. This cross-sectional study aims identify clinical features associated impairment OABD. Methods: A total 152 participants, aged at least 50 years and diagnosed (BD) related disorders agreement Diagnostic Statistical Manual Mental Disorders, Fifth Edition, Text Revision criteria, were included divided into two subgroups based on presence/absence impairment, defined as a diagnosis Mild Neurocognitive Disorder or Major Disorder. Univariate comparisons multivariate logistic regression models performed investigate associations between variables impairment. Results: Cognitively impaired had higher prevalence otherwise specified BD/cyclothymic disorder, while BD type 2 was more common cognitively unimpaired group. Additionally, group later onset major mood episodes (p < 0.05), fewer lifetime depressive = 0.006), vascular leukoencephalopathy 0.022) dyslipidemia 0.043), lower agoraphobia 0.040), worse global functioning 0.001), psychopathology severity 0.001).
