Background/Objectives: Heart failure (HF) constitutes a complex clinical syndrome that is highly prevalent worldwide, comprises serious prognosis, and results in reduced quality of life. Exercise capacity one the most significant parameters involved prognosis HF patients. Our objective was to evaluate relationship between selected cardiopulmonary exercise testing (CPET) concentration novel biomarker sST2 group patients with heart ejection fraction (HFrEF). Methods: A 135 HFrEF enrolled this prospective cohort study. Patients were stable phase disease prior 4 weeks received optimal medical treatment. Clinical biochemical investigated. All performed maximal CPET. Results: The mean (SD) 45.5 ± 39.2 ng/mL. Based on CPET results, cut-off value (52.377 ng/mL) established, for discrimination relative peakVO2 < 12 mL/kg/min. divided into two groups according values determined an ROC curve (AUC 0.692, 95% CI: 0.567–0.816). higher 14.5 4.6 mL/kg/min, while second group, it 17.6 5.2 (p = 0.002). In ≥ 52.377 ng/mL 55.6% achieved VO2 50%. Subjects lower obtained PETCO2 0.001) pulse O2 0.01). VE/VCO2slope 0.002) increased concentration. Conclusions: protein substantially associated severity left ventricular assessed by functional through
