Objective This study investigates the correlation between Life’s Essential 8 (LE8), a cardiovascular health (CVH) metric, and frailty in middle-aged elderly individuals United States, also examining how depression mediates this relationship. Methods Participants from NHANES 2005-2018 were analyzed for correlations LE8 frailty, along with combined effects of using multiple logistic regression. Dose-response relationships assessed restricted cubic splines (RCS), mediation analysis explored depression’s role. Sensitivity subgroup analyses conducted result stability. Results The included 8,982 participants, 3,103 events. A higher score was significantly associated reduced risk adjusted odds ratios medium high CVH groups at 0.49 (95% CI: 0.40-0.58, p < 0.001) 0.21 0.13-0.33, 0.001), respectively. RCS model showed negative dose-response No significant association found depressed population, where mediated 32.13% Conclusion Maintaining good health, as measured by LE8, is key to reducing individuals. underscores importance assessments targeted programs improve quality life.
