Abstract Background Post-traumatic stress has been identified as a frequent long-term complication in survivors of critical illnesses after sepsis. Little is known about trajectories post-traumatic and potentially modifiable risk factors following the ICU stay. Study objective was to explore compare different clinical symptoms sepsis up two years discharge from ICU. Methods Data on by means Symptom Scale (PTSS-10) were collected at one, six, 12 24 months chronic psychiatric diagnoses prior derived primary care provider’s health records, data intensive treatment documentation. Trajectories ex post, discriminating patterns change k-means clustering. Assignment predicted multinomial log-linear models. Results At months, all follow-up measurements PTSS-10 completed N = 175 patients. Three clusters could be regarding PTSS levels: stable low ( 104 patients [59%]), increasing 45 [26%]), recovering 26 [15%]). Patients with initially high more likely show decrease (OR 95% CI: 1.1 [1.05, 1.16]). Females 2.45 [1.11, 5.41]) reporting early traumatic memories 4.04 [1.63, 10]) higher for levels. Conclusion relevant burden Identification three within highlights importance observation, quarter reports few yet an increase following. Regular screening should considered even particular females

Abstract Being dismissed or disparaged by medical professionals can be shocking and demoralizing for patients, leading to unnecessary harms (e.g., avoidance of treatment; depression; shame). This difficulty is further compounded when patients are also the target wide-ranging stigmatization due nature behavioral diagnoses they carry, and/or social identities attributed them providers. Incorporating both clinical psychology ethics perspectives, two cases presented demonstrating insidious incurred from traumatic experiences like those described above. Both shown illustrate role that a particular form gaslighting– gaslighting – plays in creating conditions result trauma . The term “medical gaslighting” currently lacks clear stable definition larger literature; authors develop establish here. case Alex offered, illustrating standard recognized literature, significance epistemic injustice preparing grounds Next, Kiara considered, order demonstrate how becomes vehicle more subtle trauma, not captured diagnostic criteria. claims here testable, suggest work expanding conceptual reach incorporating concept into practice, warranted.

Post-traumatic stress disorder (PTSD) is a common mental health disorder, with an incidence of up to 12.5% among primary care patients. Most often, PTSD detected in combat veterans, victims terrorist attacks and terror, but it can also be consequence traumatic brain injury medical interventions. Impaired cognitive functioning key feature PTSD, including attention deficits reduced processing speed, executive dysfunction, impairments verbal learning memory. Cognitive are significantly persistent largely similar nature neuropsychological neurodegenerative pathology. Possible pathogenetic mechanisms underlying the development neuroinflammation, oxidative decreased production neurotrophic factors. One promising areas treatment use Cerebrolysin, which has powerful anti-inflammatory activity.

Great disparity is observed among studies investigating the prevalence of PTSD after burns. This systematic review and meta-analysis aimed to explore pooled in adult burn survivors over first two years post-burn. Five electronic databases were searched for observational assessing symptoms Meta-analysis was performed using an auto-correlation hierarchical effects model estimate course rates time establish point-prevalences. The effect different moderators tested with meta-regression. Thirty-two included meta-analysis. overall 20.5% (95% CI 16.4-24.6) significantly decreased by about 0.37% per month post-burn time. Questionnaire-based studies, published from 2000 onwards, more likely show a decrease compared diagnostic before 2000. A qualitative comparison revealed that inter-continental differences are be complex multi-factorial. affects one five survivors, moderately decreasing six months onwards. Early screening identification who require specialist psychological care vital burns services.

Purpose of review Dysfunction fear memory systems underlie a cluster clinically important and highly prevalent psychological morbidities seen in perioperative critical care patients, most archetypally posttraumatic stress disorder (PTSD). Several sedative-hypnotics analgesics are known to modulate systems, it is theoretically plausible that clinical decisions the anesthesiologist could impact outcomes. This aims provide focused synthesis relevant literature from multiple fields research. Recent findings There evidence some contexts unconscious less sensitive anesthetics than conscious systems. To understand how anesthetics with different molecular mechanisms affect consciousness, we explored subjective experiences recalled after responsive and unresponsive sedation induced equisedative doses of dexmedetomidine, propofol, sevoflurane, S-ketamine in healthy male participants (N = 140). The were administered experimental setting using target-controlled infusion or vapouriser for one hour. Interviews conducted anesthetic administration revealed that 46.9% (n 46) arousable 98) reported experiences, most frequently dreaming memory incorporation the setting. Participants receiving dexmedetomidine often while multimodal experiences. Responsiveness at end did not prevalence content These results demonstrate during are common agents action may have effects on complexity albeit present sample differences between drugs minute.

Very little is known regarding the impact of post traumatic stress disorder (PTSD) on course multiple sclerosis (MS).To explore pre-existing PTSD MS relapses, magnetic resonance imaging (MRI) activity, and disability in a large population-based cohort.Military Veterans with prior to symptom onset (MSPTSD, n = 96) were identified using Department Affairs databases. MSPTSD cases matched controls without (n 95). Number number new T2 lesions gadolinium brain MRI, neurological abstracted between 2015 2019.The mean annualized relapse rate was greater group versus (0.23 vs 0.06, respectively; p < 0.05), as gadolinium-enhancing MRI (0.52 0.16 0.29 0.08, 0.05). Disability accrual (time Status Scale 6.0) more rapid (23.7 29.5 years, 0.05) relapsing patients PTSD.Patients have higher disease activity reach endpoints rapidly than controls. This first study show potentially modifiable risk factor for disability.
