Background: Aging is associated with musculoskeletal pain and postural adaptations which may affect functionality. This study aims to analyse the effect of a combined protocol self-assisted manual therapy high-intensity walking on back pain, functionality, shoulder posture in older adults, establish short- medium-term effects this program. Methods: A multicentre, double-blind, randomized trial was conducted adults. The sample divided into two groups: plus at group (MTWG) Control Group (CG), latter undergoing supervised only. Pain (Pressure Threshold Visual Analogue Scale), functional capacity (5-times sit-to-stand test), change thorax position (acromion position) were assessed T0 (baseline), T1 (after 4-week intervention) T2 (follow-up, 4 weeks after end intervention). Results: total 95 adults (41 MTWG 54 CG) completed analyzed. After isolating correlations among our primary outcomes, analysis revealed statistically significant between-subject (p < 0.01), within-subject 0.001) between-within subject differences 0.05) Pressure scores favour MTWG. We also detected within-subjects for Scale, These patterns results remained stable T2. 5-times test 0.01) acromion improved but not Conclusions: walking, compared alone, short term (over one month), reduction maintained medium (at one-month follow-up).
